摘 要:目的探讨肝细胞癌(HCC)超声造影灌注参数与病理分级的相关性。方法47例HCC行术前超声造影(CEUS),其中43例行脱机时间-强度曲线(TIC)软件分析,将由曲线获得病灶内及周边肝组织感兴趣区(ROI)的灌注参数与术后标本的病理分级进行对比、分析。结果47例HCC病理分级与肿瘤的大小无关(P>0.05);43例HCC病灶内与周围肝组织ROI的灌注参数均有显著性差异(P<0.05);43例HCC病灶内ROI的造影剂增强时间、效应持续时间、增强斜率在不同病理分级的HCC间有显著差别(P<0.05),但峰值强度、到达时间无明显差异(P>0.05)。结论CEUS灌注参数与病理分级具有较高的相关性,可据此初步判断HCC的病理分级。Objective To explore the correlation of contrast-enhanced ultrasonography (CEUS) with pathologic grading in HCC. Methods CEUS was performed before operation in 47 patients with HCC. Time-intensity curves (TIC) of inner and outer regions of the lesions in 43 cases with HCC were analyzed. Pathologic grading for HCC was determined according to the scale of Edmonson. The perfusion parameters obtained from the TIC were analysed to explore the relationship between the perfusion and pathologic grading in HCC. Results No significant differences were found between pathology and the tumor size in 47 HCC (P〉 0.05); The perfusion parameters obtained from the TIC had significant differences between the tumors and normal liver parenchyma (P〈0.05); Pathologic grading of HCC was correlated to contrast-enhanced time, slope and duration time (P〈0.05), but it was not correlated to the arrival time and PI (P〉 0.05) .Conclusions Some of the perfusion data on CEUS were correlated with pathologic grading in HCC, thus CEUS offers a new way to judge the pathologic grading in vivo.
