摘 要:目的:探讨宫颈上皮内瘤变(CIN)和宫颈癌中端粒酶的活化与HPV16、18感染的关系。方法:采用免疫组化SP法检测52例CIN和69例宫颈癌中端粒酶催化亚单位(hTERT)的表达,以原位杂交法检测HPV16、18的表达,并以18例正常宫颈组织为对照。结果:正常宫颈组织中hTERT无表达,HPV16、18阳性率5.6%(1/18);CIN中hTERT阳性率50%(26/52),HPV16、18阳性率51.9%(27/52),CIN组与对照组比,两者差异均有非常显著性(P<0.01);宫颈癌组hTERT阳性率87.0%(60/69),HPV阳性率84.1%(58/69),宫颈癌组与CIN和对照组比,两者差异均有非常显著性(P<0.01)。hTERT与宫颈癌分期、组织来源、细胞分化及有无淋巴转移等均无关。无论是CIN还是宫颈癌,hTERT阳性者HPV16、18感染率均明显高于hTERT阴性者,相比差异均有非常显著性(P<0.01)。结论:端粒酶的活化与宫颈癌的发生有关,不论是CIN还是宫颈癌阶段端粒酶表达率的高低均与HPV感染率高低呈正相关关系,HPV16、18可能是端粒酶活化的始动因素。Objective: To investigate the activation of telomerase in women with CIN or cervical carcinoma and the relationship existed between the telomerase and infection of HPV16/18. Methods:52 cases of cervical intraepithelial neopiasia (CIN) and 69 cases of cervical carcinoma were detected hTERT expression by immunohistochemistry (SP) and expression of HPV16/18 by in situ hybridization. 18 normal cervical tissues were as the controls. Results:There was no hTERT expression in normal cervical tissues and the positive rate of HPV16/18 was 5.6%(1/18). The positive rate of hTERT and HPV16/18 in CIN was 60%(26/52) and 51.9%(27/52), that had significant difference when compared to the controls( P 〈 0.01 ). The positive rate of hTERT and HPV16/18 was 87.0% (60/69) and 84.1% (58/69)in cervical carcinoma group which had significant difference with that in CIN group and control group ( P 〈 0.01 ). hTERT had no relationship with tumor staging, tissue source, cell differentiation, and lymphatic metastasis. The HPV16/18 infectious rate in positive hTERT cases was obvious higher than hTERT negative cases ( P〈 0.01 ) .Conclusions:The activation of telomerase associates with the occurrence of cervical carcinoma. Either CIN or cervical carcinoma, the expression rate of telomerase shows positive correlation with the infective rate of HPV, thus we consider HPV16/18 may the initiation agent of the activation of telomerase.
