摘　　要：目的探索社区0～12月龄儿童发展干预模式,促进早期儿童发展。方法2005-06—2006-08在北京市城镇12个保健科采取病例对照、纵向追踪、随机整群抽样的研究方法,共收集数据210份,试验组66例、对照1组74例、对照2组70例。按各组的工作流程追访1年(6个月以内1个月1次,6～12个月每2个月1次)。采用Gesell发育诊断法、HOME量表、依恋问卷评估干预模式效果。结果试验组的安全依恋行为(F=4.35,P<0.05)、父母提供适宜玩具(F=5.37,P<0.01)、丰富环境刺激(F=5.87,P<0.01)与对照1组、2组比较差异均有显著性,在家居组织环境(均数差=0.49,P<0.01)方面好于对照1组,在语言发育(均数差=5.12,P<0.05)、个人-社会能力发展(均数差=7.57,P<0.05)方面好于对照2组;试验组及对照1组的保健费用低而保健模式满意率高于对照2组。结论综合干预模式能够通过改善父母的养育知识及行为,促进早期儿童发育及社会性的发展;具有发育监测作用,适宜在社区儿童保健中使用。Objective To explore the integrated intervention pattern for infant care and development, which is suitable for the community child health care and to promote the early child development. Methods The control vertical trial and random cluster sample were adopted in study. Altogether 210 neonates were followed up for 12 months and they completed all measures. The infant development of the intervention pattern was assessed by standardized Gesell developmental schedule, HOME inventory and The Attachment Q-sort, and its benefit was evaluated by the family expenditure and satisfaction. Results Compared with the contrast pattern, the infants managed by integrated intervention had more satisfactory out- comes, having more secure attachment behaviors （ F = 4. 35, P 〈 0. 05）, more age-suitable toys （ F = 5.37, P 〈 0. 01） and a plenty of environmental stimulations （ F = 5.87, P 〈 0. 01） offered by their parents. Their families had lower expenditures and more satisfied than others. Conclusion The integrated intervention can improve parent＇s fostering behaviors and knowledge. It is suitable for promoting the early child development in the community health center.
