RGP对大龄儿童屈光参差性弱视的矫治作用探讨

摘　　要：目的：探讨硬性透气性角膜接触镜（rigid gas-permeable contact lenses．RGP）在大龄儿童屈光参差性弱视矫治中的效果和安全性。方法：选择在我院配戴RGP的28例（28眼）9～16岁屈光参差性弱视儿童作为RGP组，并以配戴框架眼镜的28例（28眼）条件相似的儿童作为对照组。两组均辅以正规弱视综合治疗，观察6too，分别对RGP组首次配戴RGP前后的矫正视力及两组1，3，6mo后的视力变化、疗效进行统计学比较，同时观察RGP组的不良反应及并发症。结果：RGP组的框架眼镜矫正视力平均为4．38±0．41，首次配戴RGP当dRGP矫正视力平均为4．60±0．22，差异有显著性（t=10．11，P〈0．01）；经弱视治疗后1，3，6mo后，RGP组视力改善明显好于对照组，总有效率RGP组分别为68％，82％，86％，对照组分别为7％，21％，28％，两组同期疗效比较均具显著性差异（P〈0．01，χ^2检验）。在随访期间，未发现角膜上皮损伤或角膜感染等严重并发症。结论：RGP矫治大龄儿童屈光参差性弱视具有积极的临床意义，与框架镜相比具有明显优势，为弱视治疗提供一种安全、有效、快速的矫治手段。AIM： To explore the clinical effects and safety of using rigid gas-permeable contact lenses （RGP） for anisometropic amblyopia in older children. METHODS： Twenty-eight older children （28 eyes） with anisometropic amblyopia, aged 9-16 years old, were selected to wear RGP, and another 28 counterparts （28 eyes） were selected as controls by wearing spectacles. Both groups received regular comprehensive therapy for amblyopia. The study was done over 6 months with follow-up visits every one month. Visual acuity of two groups was observed and compared statistically, mtatisticaaly, meanwhile the adverse, reactions and complications were observed in RGPgroup.RESULTS： In RGP group , when children wore RGP for the first time , The corrected visual acuity with RGP was （4.60 ±0.22）, better than that with spectacles （4.38 ±0.41 ）. The result was statistically significant（ P〈 0.01, t-test）. 1, 3and 6 months later, the total effective rate in RGP group was 68%, 82% and 86% respectively, while that in control group was 7%, 21% and 28%. The enhancement of visual acuity in RGP group was remarkably better than that in control group. There was a significant difference between two groups（ P〈 0.01 ,χ^2-test） over the same time. In RGP group, no serious complications such as corneal abrasion and inflammation occurred within follow-up period. CONCLUSION： RGP are superior to spectacles because it can provide better optic corrections and better corrected visual acuity. RGP has good therapeutic effects and is a safe, effective and rapid approach to improve visual acuity for anisometropic amblyopia.
