显微血管减压术治疗46例面肌痉挛

摘　　要：目的探讨显微血管减压术治疗面肌痉挛(HFS)的注意事项及方法。方法经过磁共振断层血管成像检查后,对有血管压迫者的46例面肌痉挛患者行显微血管减压术治疗,随诊比较疗效,分析手术操作与手术并发症的关系。结果46例病例中术后症状立即消失25例,1w内消失10例,1个月内消失6例,半年内消失1例,观察半年4例症状减轻,无复发病例。术后3例发热,2例脑脊液漏,9例有迟发性面瘫,1例听力丧失,2例轻微共济失调。结论显微血管减压术是治疗面肌痉挛的有效方法,正确的手术方式有助于减少手术并发症。Objective To explore the crucial factors in the treatment of hemifacial spasm （HFS） with microvascular decompression. Methods After scanned by the magnetic resonance tomographic angiography, 46 patients suffered from HFS whose symptoms mainly caused by the compression on vascular were performed microvascular decompression respectively. The therapeutic effect of each case was investigated and compared in the follow-up studies. The relationship of operative procedure and the relative complications was analyzed, Results In all the patients, the symptoms of 25 cases disappeared completely immediately after the operation, as well as 17 cases of experienced delayed resolution of residual hemifacial spasm after the operation （ 10 cases in a week, 6 cases in a month, and 1 case in half a year）. The symptoms of 4 cases lightened after the surgery and remained stable during half a year. No recurrence was in all cases. As to the complications caused by the surgery, the fever occurred in 3 cases, the leak of cerebrospinal fluid in 2 cases, the tardive facial palsy in 9 cases, hearing disability in 1 case, and slight ataxia in 2 cases. Conclusion Microvascnlar decompression is the effective method to treat hemifacial spasm, The correct way of performing the surgery contributes to the decrease of complications directly caused by the surgery.
