摘 要:目的分析腹膜后神经源性肿瘤的MRI表现,以便提高本病的诊断准确性。方法对22例经手术和病理证实的腹膜后神经源性肿瘤MRI表现进行回顾性分析,并与手术病理对照。结果22例神经源性肿瘤均位于肾上腺区或沿交感神经链分布。良性肿瘤16例,肿块平均直径为3.6±1.4cm,形态规则;恶性肿瘤6例,肿块平均直径为9.4±1.2cm,形态不规则。成神经细胞瘤T2WI表现为明显高信号内夹杂低信号的网格影、血管包埋;嗜铬细胞瘤T2WI有脑脊液样高信号,增强后明显强化;神经鞘膜瘤常合并囊变。对于良、恶性肿瘤的鉴别,MRI信号强度的价值不大,而肿瘤的形态及大小更有意义。结论结合腹膜后神经性肿瘤的临床表现和病理特点,MRI可对多数腹膜后神经源性肿瘤作出准确的诊断。Objective To analyze the MRI findings of retroperitoneal neurogenic tumors so as to raise the diagnostic accuracy of this disease. Methods MRI findings of 22 patients with surgically and pathologically proved retroperitoneal neurogenic tumors were retrospectively analyzed and compared with those of surgery and pathology. Results All retroperitoneal neurogenic tumors in 22 cases were located on the adrenal area or were distributed along the sympathetic ganglionic chain. Among 22 cases, benign tumors were found in 16, whose mean diameter was 3.6 ± 1.4 cm, while the shape was regular, malignant tumors were found in 6, whose mean diameter was 9.4 ± 1.2 cm, while the shape was irregular. Neuroblastomas showed obvious hyperintense on T2WI mixed up hypointense reticular shadows, vascular embed and retroperitoneal lymphonodi enlargement. Pheochromocytomas showed hyperintense similar to cerebrospinal fluid on T2WI and marked enhancement after enhanced scans. Neurilemmomas frequently associated with cystic change. For the differentiation between benign - and malignant tumors, MRI signal intensity was not rcliable evidence, whereas tumoral shape and size were more important reference. Conclusion The integration of MRI findings with clinical manifestations and pathologic examinations can accurately diagnose most retroperitoneal neurogenic tumors.
