摘 要:目的:研究周围型肺癌肺肿瘤CT灌注参数与PET的标准摄取值(SUV)及患者预后的相关性。方法:42例有同期PET的周围型肺癌行肺肿瘤灌注CT扫描,病例均经病理证实并追踪1年以上。扫描数据用CT灌注2——体部肿瘤软件包进行分析。结果:42例周围型肺癌中,病灶强化差值14~63HU,平均(32.80±12.35)HU;PET的最大SUV值2.76~18.50,平均8.63±3.47;肿瘤灌注的血容量值(BV)为6.0~17.8ml/100g,平均(8.47±2.60)ml/100g;毛细血管通透性(PS值)为5.95~95.00ml/(100g.min),平均(49.13±17.33)ml/(100g.min)。PET和肺灌注的BV值对肺癌的诊断最为敏感,敏感度达100%,PS和CT强化值敏感性稍差,敏感度分别为95%和91%。42例中,CT强化值与PS值、SUV值和预后无相关性,r值分别为0.053、0.212和0.07,P值均>0.05;BV值与强化值有很好的正相关性,r值为0.341,P<0.05,SUV值和PS值与预后有明显的正相关性,r值分别为0.319和0.378,P值分别为0.039和0.014,均<0.05,且PS值与SUV值也有正相关性,r值为0.318,P<0.05。结论:BV有较高的诊断敏感性,但与预后没有明确的相关性;PS诊断敏感性稍差,但其数值与PET的SUV值和患者的预后正相关,PS值与SUV值共同反映肿瘤的恶性特征。Objective:To investigate the correlation between CT perfusion parameters,SUV of PET and prognosis of patients with peripheral lung cancer. Methods: 42 patients with peripheral lung cancer diagnosed by pathology and with more than 1 year follow-up were included in this study. All of the patients underwent both CT perfusion and PET scanning. CT perfusion was analyzed using software package of CT Perfusion 2. Results:The enhancement difference in 42 cases was between 14-63HU with a mean difference of (32.8±12.35)HU. The maximal SUV value in 42 cases ranged from 2.76-18.50 with a mean value of 8.63±3.47. On CT perfusion,blood volume (BV) of the tumors was 6-17.8ml/100g with a mean value of (8.47±2.60)ml/100g,and Permeability surface (PS) ranged from 5.95-95.00ml/(100g·min) with a mean value of (49.13±17.33)ml/( 100g· min). The sensitivity for diagnosis of lung cancer of both PET and BV value in CT perfusion was 100%,and 95% for PS,91% for CT enhancement difference. CT enhancement difference had no association with PS, SUV and prognosis (r=0. 053,0. 212,0. 07, respectively, P〉0. 05) ; BV was associated with CT enhancement difference (r=0. 341,P〈0.05),SUV and PS of PET was significantly correlated with the prognosis (r=0. 319, P= 0. 039 and r=0. 378,P=0. 014 respectively P〈0.05), and PS was associated with SUV (r= 0. 318,P〈0.05). Conclusion.. BV has high sensitivity for diagnosis of lung cancer,and it was associated with CT enhancement difference, but not with prognosis. Compared with BV,PS has less sensitivity for diagnosis of lung cancer,but it was positively correlative with SUV and prognosis. Combination of PS and SUV can reflect the malignant feature of lung cancer.
