改良准分子激光上皮瓣下角膜磨镶术治疗放射状角膜切开术后残余近视和散光的临床观察

摘  要:目的评价改良LASEK治疗RK术后残余近视和散光的有效性和安全性。方法对15例26眼RK术后5~7年残留近视及散光的患者进行LASEK治疗,并用改良上皮瓣的制作办法(撕囊法),所有术眼术前裸眼视力≤0.3,术后随访2年。结果平均屈光度由术前(-4.37±1.24)D下降至术后的(-0.43±0.24)D,平均散光度由术前的(-1.57±1.34)D下降至术后的(-0.32±0.30)D。24眼术后裸眼视力≥术前的最佳矫正视力,22眼术后裸眼视力≥1.0,2眼≥0.8,有2眼回退,回退度数<-1.25D。结论改良LASEK是一种安全有效的矫正RK术后残留近视散光的方法。[Objective] To evaluate the safety and efficacy of laser subepithelial keratomileusis (LASEK) with improved making epithelial cap for treatment of undercorrection after radial keratotomy (RK). [Method] 26 eyes that had undergone radial keratotomy at least 5 years ago were corrected with laser subepithelial keratomileusis ( LASEK).Using an improved method (tear pouch method) to make an epithelial cap. The corrected visual acuity for these 26 eyes was 0.3 or low. The average follow-up period was 2 years. [Result] Mean refractive sphere was reduced from(-4.37±1. 24)DS preoperatively to (-0.43±0.24) DS postoperatively. Mean refractive cylinder was reduced from (-1.57 ±1.34) DC preoperatively to (-0. 32 ±0. 30) DC postoperatively. The uncorrected visual acuity (UCVA) of 24 eyes reached or exceeded their preoperative corrected visual acuity. The UCVA of 22 eyes reached 1.0 and 2 eyes reached 0. 8,with undercorrective refractive sphere〈-1.25D. [Conclusion] The correction of undercorrection after radial keratotomy(RK) by LASEK with improved making epithelial cap was proven to be safe and effective.
