弧形切割吻合器在低位直肠癌保肛术中的应用初探

摘　　要：目的:评价弧形切割吻合器(凯途)在低位直肠癌保肛手术中的临床价值。方法:回顾分析2005年10月至2006年6月期间,18例患者在低位直肠癌保肛手术中应用弧形切割吻合器的临床资料。结果:18例患者均使用双吻合器行一期吻合。术后10d出现肠瘘1例(5·6%),术后8d出现直肠阴道瘘1例,1个月瘘口均未愈合而行横结肠造瘘。肛门坠胀不适6例,里急后重13例;日排便次数为5～10次8例,10次以上1例。1例出现严重尿储溜需放尿管。3个月后排便和排尿功能恢复接近正常。结论:弧形切割吻合器进行保肛手术安全可靠,操作简便,对患者术后排便、排尿功能影响较小。Objective： To evaluate the clinical value of curved-cutter-stapler in anus-preserving operation for low rectal cancer. Methods： The clinical data of 18 patients with low rectal cancer, received anus-preserving operation using curved-cutter-stapler from 2005 to 2006 were reviewed retrospectively. Results： One patient had anastomotic leakage 10 days after operation and one patient had rectovaginal fistula 8 days after operation. Bowel dysfunction was found in the half of these patients, and there was one bladder dysfunction, but bowel and bladder function recovered about three months later. Conclusion： Curved-cutter-stapler is safe, reliable and easy to use in anus-preserving operation for low rectal cancer, with less bowel and bladder disfunction.
