摘  要:目的探讨事件相关电位(ERP)非匹配负波(MMN)评价阿尔茨海默病(AD)患者认知水平及监测AD病情的价值。方法采用肌电诱发电位仪对30例AD患者及同期30例健康受试者进行MMN检测。结果实验组MMN峰潜伏期(PL)为(270.90±42.96)ms、波幅(Amp)为(2.72±1.24)μv,对照组PL为(211.27±19.17)ms,Amp为(5.18±1.99)μv,两组差异有显著性意义(t=6.709,P(0.01;t=-5.609,P(0.01);中、重度AD患者MMN(PL、Amp)与轻度患者差异均有显著性意义(P(0.01),而中度与重度AD患者间差异无显著性意义(P>0.05)。结论MMN可反映AD患者的认知水平,还可对AD病情进行监测。Objective To explore the value of event-related potentials (ERP) and mismatch negativity ( MMN ) to appraise Alzheimer's disease (AD) patient's cognition and monitor AD condition. Methods Thirty patients fulfilling the NINCDS - ADRDA criteria of AD were diagnosed by two specialized doctors. The sex, age and education-matched 30 healthy subjects were also chosen as controls. The inspection of MMN was carried on with the help of myoelectricity and evoked potentials inducer. Results The MMN PL of AD patients was significantly prolonged compared with control group[ ( 270. 90±42. 96 ) ms versus ( 211.27±19. 17) ms, P 〈 0. 01 ], and Amp of AD patients was decreased compared with control group [ ( 2. 72 ±1.24 )μv versus (5.18±1.99)μv, P 〈0.01 ]. The MMN PL of AD patients with moderate and severe dementia was signifieantly prolonged eompared with mild group [(282.92±20.61)ms versus (223.89±32. 52)ms , P 〈 0.01;(301.89±35.57)ms versus ( 223. 89±32. 52)ms, P 〈 0. 01 ], and Amp was deereased compared with mild group [(2. 48±1.08)μv versus (3.97±0.47)μv,P 〈0.01 ; ( 1.79±0.98)μv versus (3.97±0.47)μv,P 〈 0.01 ]. There was no signifieant differenee of MMN between moderate group and severe group( P 〉 0. 05). Conclusion MMN ean refleet the eognition level of AD patients and monitor the alteration of patients' condition.
