非小细胞肺癌中P27kip1蛋白的表达与细胞增殖及预后的关系

摘　　要：目的研究P27kip1在非小细胞肺癌(NSCLC)中的表达,探讨P27kip1表达缺失的临床意义及其与肺癌细胞增殖和预后的关系。方法SP免疫组化法检测68例非小细胞肺癌的P27kip1及Ki-67表达情况。采用χ2检验、Kaplan-Meier法、Log-rank曲线和Cox比例风险回归等方法分析P27kip1表达缺失的意义及与预后的关系,t检验分析p27表达与Ki-67标记指数的相关性。结果68例NSCLC中p27表达的缺失率为51.5%,p27表达的缺失与性别、年龄、肿物大小、组织学分型等无关(P>0.05),而与Ki-67标记指数、临床分期、分化程度、淋巴结转移有关(P<0.05)。P27kip1阴性的患者ki67标记指数高于P27kip1阳性的患者。单因素生存分析表明,p27表达的缺失与肺癌的生存率密切相关,p27阳性患者的生存率显著高于p27阴性患者(P<0.05)。多因素分析结果显示,肺癌的淋巴结转移和p27表达的缺失可以作为判断肺癌预后的危险因素。结论p27的表达的缺失与NSCLC的细胞增殖、去分化、临床分期、淋巴结转移及预后密切相关,可以作为估计生存率的独立指标。Objective To investigate the expression of P27Kip1 in non-small cell lung carcinoma（NSCLC）and its association with cell proliferation, clinicopathological variables and prognosis. Methods P27Kip1 and ki67 expressions were determined in 68 patients with NSCLC by immunohistochemical technique. Results Loss of expression of P27Kip1 （51.5%）was observed in 60 cases of NSCLC. The P27Kip1 expression was not related to age,sex, tumor size, and histological subtypes （P 〉 0.05） and closely related to ki67 index, differentiation, lymphatic metastasis, and clinical stage （ P 〈 0.05 ）. The ki67 index in P27Kip1 negative cases （ 32.70 ± 21.23） was higher than that in P27Kip1 positive cases（22.06 ± 19.47）. Patients with positive P27Kip1 showed significantly worse survival rates than patients with negative （ P = 0. 0222）. The loss of P27Kip1 expression correlated with markedly poor prognosis（ P 〈 0.05）. Multivariate analysis indicated that lymph node involvement and P27Kip1 expression were independentpregnostic factors in cases of NSCLC. Conclusion P27Kip1 expression contributed to differentiation, lymphatic metastasis, and clinical stage. P27Kip1 expression was related to the prognosis, and could be significant independent poor prognostic marker in the NSCLC. P27Kip1 expression be important in regulating cell proliferation in lung cancer.
