摘 要:目的:探讨膀胱癌组织中凋亡抑制基因存活素(Survivin)和血管内皮生长因子(VEGF)的表达与膀胱癌血管生成的关系。方法:应用免疫组化S-P法检测69例膀胱癌组织和8例正常膀胱粘膜组织(对照组)中Survivin和VEGF的表达,同时分析不同Survivin和VEGF表达状态下膀胱癌组织微血管密度(MVD)的变化。结果:膀胱癌组和对照组Survivin阳性率分别为71.0%(49/69)和0(0/8),二组比较差异亦有统计学意义(χ2=11.31,P<0.01);膀胱癌组和对照组VEGF阳性率分别为66.7%(46/69)和0(0/8),二组比较差异有统计学意义(χ2=10.62,P<0.01);膀胱癌组织中Survivin和VEGF的表达呈正相关(r=0.785,P<0.01)。将膀胱癌分为4组,A组(n=39)为Survivin+/VEGF+,B组(n=13)为Survivin-/VEGF-,C组(n=7)为Survivin-/VEGF+,D组(n=10)为Survivin+/VEGF-。4组MVD分别为46.69±11.28、30.72±5.10、36.18±8.62、34.47±7.98,A组MVD显著高于其他3组(P均<0.01)。结论:膀胱癌中Survivin可能通过信号传导网络参与调控VEGF的表达而促进血管生成,导致肿瘤恶性进展。Objective:To study the relationship between expression of survivin and vascular endothelial growth factor (VEGF) and angiogenesis in bladder cancer. Methods:By immunohistochemical S-P method, epressions of survivin and VEGF were examined in 69 bladder cancer samples and 8chronic cystitis samples (control group); and the microvessel density (MVD) was analyzed among different expression states of survivin and VEGF in bladder cancer tissues.Results:The positive rates of survivin in bladder cancer was 71%(49/69) which was significant higer (χ^2=11.31,P〈0.01) than that in control group 0%(0/8). The positive rates of VEGF in bladder cancer was 66.7%(46/69) which was significant higer (χ^2=10.62,P〈0.01) than that in control group 0%(0/8). There was a positive correlation between survivin and VEGF expression (r=0.785, P〈0.01). The bladder cancer group was subdivided into 4 groups: survivin-negative and VEGF-negative cases as groupA (n=39), survivin-negative and VEGF-negative cases as groupB (n=13), survivin-positive and VEGF-negative cases as group C(n=7), survivin-positive and VEGF-negative cases as groupD(n=10). The meas MVD of groups A, B, C, and D were 46.69±11.28, 30.72±5.10, 36.18±8.62 and 34.47±7.98, respectively. The mean of MVD in group A was significantly higher than other three groups(P〈0.01). Conclusion:Expression of survivin gene in bladder cancer may promote angiogenesis through increasing in VEGF expression, and thus induce tumor progression.
