胸腔闭式引流顺铂和IL-2治疗肺癌胸水的临床观察

摘　　要：目的:观察胸腔闭式引流顺铂和IL-2治疗肺癌胸腔积液的疗效和安全性。方法:1999年8月至2005年6月,以胸腔闭式引流顺铂和或IL-2治疗肺癌胸腔积液57例,随机分成治疗组27例,对照组30例,胸水排净后,治疗组胸腔内注入顺铂40 mg^60 mg和IL-2 100万U,对照组胸腔内注入顺铂40 mg^60 mg,4周后观察胸水。结果:治疗组完全吸收7例,部分吸收19例,无效1例,总有效率96.3%;对照组完全吸收5例,部分吸收16例,无效7例,总有效率74%,经统计学分析,具有统计学意义(P<0.05)。主要毒副反应,两组均为Ⅰ~Ⅱ度,无统计学意义。结论:胸腔闭式引流顺铂和IL-2治疗肺癌胸水疗效较好,毒副反应较轻。Objective： To evaluate the efficacy and safety of thoracic closed drainage combined with cisplatin and IL-2 for pleural effusion of pulmonary carcinoma.Methods： Fifty-seven patients with pleural effusion of pulmonary carcinoma were randomly divided into two groups（treatment group,27 patients;control group,30 patients） to receive the treatment of thoracic closed drainage combined with cisplatin and/or IL-2 from August 1999 to June 2005.After drainage,cisplatin 40 mg～60 mg and IL-2 1 million units were injected into pleural cavity of the patients in treatment group and cisplatin 40 mg～60 mg in control group.The pleural effusion absorption was observed after 4 weeks.Results： There were 7 cases of complete absorption,19 cases of incomplete absorption and 1 case of inefficacy in treatment group.The total effective rate in treatment group was 96.3%.And there were 5 cases of complete absorption,16 cases of incomplete absorption and 7 cases of inefficacy in control group.The total effective rate in control group was 74%.There were significant differences between the two groups（P〈0.05）.The main toxicity and side effects in two groups were degree I-II and there were no significant differences between the two groups.Conclusion： Combination of thoracic closed drainage combined with cisplatin and IL2 for pleural effusion of pulmonary carcinoma has a better effect and fewer toxicities.
