摘　　要：目的:调查我院近3年下呼吸道革兰阴性杆菌的耐药情况。方法:使用MIC法对我院下呼吸道感染住院患者的痰液标本中临床分离的603株革兰阴性杆菌进行药敏试验,并用WHONET5进行数据分析。结果:603株革兰阴性杆菌中最常见的菌种依次为铜绿假单胞菌(34.7%)、大肠埃希菌(17.4%)、鲍曼不动杆菌(15.4%)、肺炎克雷伯菌(10.9%)。铜绿假单胞菌对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦及头孢他啶耐药率低于30%;鲍曼不动杆菌耐药情况比较严重,除对亚胺培南和头孢哌酮/舒巴坦较敏感外,对头孢吡肟的耐药率为64%,对其余所检测药物的耐药率均在80%以上;大肠埃希菌对亚胺培南无耐药菌株检出,对阿米卡星的耐药率为20.8%,对其余所检测药物的耐药率均在60%以上。结论:本研究结果对我院革兰阴性杆菌感染的经验用药治疗有重要参考价值。OBJECTIVE： To investigate the bacterial resistance of gram - negative bacilli isolated from lower respiratory tract in our hospital. METHODS： Antimicrobial susceptibility of 603 gram - negative bacilli isolates collected from the sputum specimen of the inpatients in our hospital who had lower respiratory tract infection was tested by MIC method, and the data were analyzed by WHONET5. RESULTS： Among the total 603 gram - negative bacilli isolated, the most common species were P. aeruginosa （34.7%）, E. coli （17.4%）, A. baumannii （15.4%） and K. pneumoniae （10.9%）. The drug resistance rates of P. aeruginosa isolates to imipenem, cefoperazone/sulbactam, piperacillin/tazobactam and ceftazidimel were all less than 30%. The isolates of A. baumannii were susceptible to imipenem and cefoperazone/sulbactam, but their resistant rate stood at 64 % to cefepim and above 80 % to other test drugs. No resistance to imipenem was detected in E. coli, but the resistant rate of E. coli stood at 20.8 % to amikacin and above 60 % to other test drugs. CONCLUSION： Our data may have great significance in the empirical use of antimicrobial agents for the treatment of infections caused by gram - negative bacilli in our hospital.
