多西紫杉醇联合顺铂治疗蒽环类化疗失败晚期乳腺癌的临床研究

摘　　要：①目的探讨多西紫杉醇联合顺铂治疗蒽环类药物化疗失败的晚期乳腺癌的疗效和安全性。②方法40例蒽环类药物治疗失败的复发转移性乳腺癌患者均接受多西紫杉醇联合顺铂方案治疗，随机分为A组：多西紫杉醇75mg／m^2静滴，第1天；DDP80mg／m^2，第1～3，每3周为1个疗程；B组：多西紫杉醇37．5mg／m^2静滴第1、8天；DDP80mg／m^2，第1～3，每3周为1个疗程。每周期后评价疗效同时记录不良事件。③结果40例患者均可评价疗效，完全缓解（Ca）2例，部分缓解（PR）20例，有效率（CR＋PR）55％（22／40）。两组144个疗程可评价毒性反应，无严重不良事件导致死亡的患者，中性粒细胞减少是主要不良反应，Ⅲ-Ⅳ度占48．6％。④结论多西紫杉醇联合顺铂是治疗蒽环类药物化疗失败晚期乳腺癌的有效方案，其不良反应能够耐受。Objective To evaluate the efficacy and toxicity of docetaxel - cispltin combination on patients with metastasis breast cancer patients with anthracyclin chemotherapy failure. Methods 40 patients with anthracyclin chemotherapy failure were randomized into group A or group B. Group A was treated with docetaxel 75mg/m^2 iv ld,cisplatin 80mg/m^2 （ 1 ～3d） ;Group B was treated with docetaxel 37.5mg/m^2 iv ld and 8d,cisplatin 80mg/m^2 （ 1～3d） ,a treatment period for every 3 weeks. The effects and side effects were receded after every period. Results In 40 patients, complete response （CR） was 2 partial response（PR） was 22, the overall elinieal response rate was 55% （22/40）. The toxicity at 144 treatment periods was evaluated ,there was no serious adverse event leading to death. Main toxicity was neutrophfl decrease and the incidence of Ⅲ and Ⅳ grade was 48.6%. Conclusion Combination chemotherapy of docetaxel and cisplatin was an efficiency project in treatment of metastasis breast cancer patients with anthracyclin chemotherapy failure and its bad responses could be suffered.
