阳明热证 实证患者血浆内毒素水平和细胞因子的改变及相关性研究

摘　　要：目的：观察在阳明热证、实证的发病过程中血浆内毒素（ET）水平以及细胞因子的变化及其内在联系。方法：选择具阳明热证患者25例,具阳明实证患者15例,在针对原发病西医治疗的基础上,分别加服白虎汤、复方大承气汤,并另设健康对照组12名,各组于治疗前后测定血浆ET、血清IL-6、TNF-α、IL-10等水平的变化。结果：入院时,阳明热证、实证血浆ET、血清IL-6、TNF-α、IL-10水平均升高,与健康对照组比较差异有显著性（P〈0.01-0.05）,ET、IL-6、TNF-α、IL-10水平在阳明实证患者中高于阳明热证（P〈0.05）,经分别治疗3天后,阳明实证患者TNF-α、IL-6水平仍高于阳明热证（P〈0.05）,而ET、IL-10水平在两证型组中无显著性差异（P〉0.05）。结论：阳明热证、实证与血浆ET水平及其介导的细胞因子密切相关。Objective： To explore the changes of endotoxin（ET） in plasm and sytokines in the pyretic syndrome and the sthenia syndrome of Yang Ming syndrome and the inherence contact. Methods ： Twenty - five patients with the pyretic syndrome of Yang Ming syndrome and Fifteen patients with the sthenia syndrome of Yang Ming syndrome were treated by Bai Hu Decoction and Potent Purgative Decoction with Additions based on west medicine to the primary affection, and a normal control group consisting of twelve subjects was also set up. We determined the endotoxin content in plasm, IL -6, TNF -et, IL - 10 in serum of patients before and after treatment. Result： Levels of ET, IL -6,TNF - α,IL - 10 in the sthenia syndrome and the sthenia syndrome of Yang Ming syndrome were higher than those in the normal control group significandy （ P 〈 0.01 - 0.05 ）. Levels of ET, IL- 6 ,TNF- et, IL- 10 in the sthenia syndrome of Yang Ming syndrome were significantly higher than those in the pyretic syndrome of Yang Ming syndrome before treatment（ P 〈 0.05 ）. After treating three days, TNF -α, IL -6 in the sthenia syndrome of Yang Ming syndrome were still significantly higher than those in the pyretic syndrome of Yang Ming syndrome （ P 〈 0.05）, ET and IL - 10 had no different in both groups（ P 〉0.05）. Conclusion： The pyretic syndrome and the sthenia syndrome of Yang Ming syndrome have close correlation with blood level of ET in plasm and cytokines in serum.
