药物自我处置技能训练程式在社区慢性精神分裂症患者康复中的应用

摘　　要：目的探讨药物自我处置技能训练对降低社区精神分裂症患者复发率，提高社会功能的作用。方法选择4个街道社区病程在5年以上的精神分裂症患者100例，随机分成药物自我处置技能训练组（简称训练组）和对照组各50例，对训练组施以药物自我处置技能训练，共12周，随访1年，对照组不参加康复训练，但提供与训练组类似的资料，采用大体评定量表（GAS）、社会功能缺陷筛选量表（SDSS）、阳性和阴性症状量表（SAPS和SANS），对2组患者分别于3个月末、1年末进行康复状态的评估，并比较患者的服药依从性、复发情况和再住院情况。结果与入组时比较，训练组GAS、SDSS、SAPS和SANS评分明显优于对照组，服药依从性高于对照组（P〈0．01），1年内训练组复发率（10％）和再住院率（17％）均明显低于对照组（32％和43％）。结论药物自我处置技能训练能够提高慢性精神分裂症患者的药物依从性，改善患者阳性和阴性症状、社会功能和社交技能，降低复发率，是当前较为有效的精神康复措施之一。Objective To explore the effects of skills training for self-medication program on the decrease of rate of relapse and increase of social function in schizophrenia patients. Methods 100 schizophrenia patients, from 4 communities and with illness course of above 5 years, were divided into 2 groups, the training group and the control group. Only the training group was given skills training for selfmedication for 12 weeks with 1-year follow-up visit. The rehabilitative effect of the two group was evaluated by the Global Assessment Scale（GAS）,Social Disability Screening Schedule（SDSS）, Scale for Assessment of Positive Symptoms（SAPS） and Scale for Assessment of Negative Symptoms（SANS） at the end of the 3rd month and the 12th month.The patient compliance, illness relapse and reemployment of the two groups were also compared. Results The scores of GAS, SDSS, SAPS and SANS and the patient compliance in the training group were significantly better than that of the control group （P 〈0.01 ）. Both of the recurrence rate （10%） and reemployment rate （17%） in the training group are lower than that in the control group（32% and 43%）. Conclusion Skill training for self-medication program can improve the medication compliance, alleviate the positive and negative symptoms, social function and communicating skills and decrease recurrence rate in schizophrenia patients and proved to be an effective measure for mental rehabilitation.
