新生血管性青光眼两种术式治疗的疗效比较

摘　　要：目的观察比较睫状体冷凝＋小梁切除术及睫状体冷凝＋小梁切除＋脉络膜上腔引流术2种手术方式治疗新生血管性青光眼的临床疗效。方法回顾性分析2002年2月至2006年1月收治的新生血管性青光眼患者37例（38眼）,运用睫状体冷凝＋小梁切除术治疗17例（17眼）,睫状体冷凝＋小梁切除＋脉络膜上腔引流术治疗20例（21眼）。术后不用或仅局部应用抗青光眼药物,眼压控制在5～21mmHg（1kPa=7.5mmHg）,且视力稳定、角膜透明者视为手术成功。结果随访时间均在1a以上,平均随访（34.3±13.7）个月。末次随访,经睫状体冷凝＋小梁切除术治疗成功10眼（58.8%）;经睫状体冷凝＋小梁切除＋脉络膜上腔引流术治疗成功17眼（80.9%）。2种手术方式手术成功率差异有统计学意义（χ^2=4.815,P〈0.05）,运用后者治疗手术成功率高。结论睫状体冷凝＋小梁切除＋脉络膜上腔引流术治疗新生血管性青光眼较睫状体冷凝＋小梁切除术设计合理,手术成功率高,并发症少,远期疗效好。Objective To compare the clinical effect between combined application of cyclocryotherapy and trabeculectomy and combined application of cyclocryotherapy,trabeculectomy and epichorodal space drainage on neovascular glaucoma.Methods Thirty-seven cases （38 eyes） diagnosed with neovascular glaucoma between 2002,Feburary and 2006,January were studied.Seventeen cases（17 eyes） were treated with combined application of cyclocryotherapy and trabeculectomy.Twenty cases （21 eyes） were treated with combined application of cyclocryotherapy,trabeculectomy and epichoroidal drainage.Definitions of successful operation were determined as：intraocular pressure（IOP） controlling 5-21 mmHg（1 kPa=7.5 mmHg） with no anti-glaucoma medication or only local medication of topical anti-glaucoma drops,vision improved,cornea cleared.Results All cases were followed-up for over one year,and the mean followed-up time was（37.3±13.7） months.According to the last follow-up,10 eyes （58.8%） were successful in the first group;17 eyes（80.9%） were successful in the latter group.Statistical significant difference was found in the successful rate between these two groups（χ^2=4.815,P〈0.05）.Conclusion The combined application of cyclocryotherapy,trabeculectomy and epichoroidal spaces drainage has a higher success rate and fewer complications during the follow-up time on treating neovascular glaucoma.
