摘  要:目的分析节细胞神经瘤的MRI表现及其血液动力学特征,以提高其诊断准确性。方法经手术病理证实的节细胞神经瘤9例,分别经MRI自旋回波T1WI、快速自旋回波T2WI、屏气扰相梯度回波T1WI动态增强扫描。仔细复习MRI扫描结果并和手术病理作回顾性对照分析。结果9例节细胞神经瘤中,脊柱旁6例(颈部2例,纵隔1例,腹膜后3例),肾上腺3例。肿瘤直径3.7~16.4cm,平均7.1cm。SET1WI为均匀低信号4例,混杂低信号5例;FSET2WI显著高信号肿瘤3例,高低信号混杂5例,等信号1例。SPGR动态增强动脉期轻微强化,门脉期和延迟期为轻度进行性延迟强化。强化方式包括线条交织状强化6例,漩涡状强化2例,斑点状强化1例。3例肿瘤内血管强化。所有肿瘤境界清楚,瘤体自身受压变形7例,形成伪足样尖角4例。5例肿瘤推移邻近血管,所有与肿瘤解剖关系密切的血管形态正常。结论节细胞神经瘤MRI征象具有特点,动态MR可提高诊断率。Objective To explore the dynamic MR findings of ganglioneuromas ,so that to improve its diagnostic accuracy. Methods 9 cases of ganglioneuromas confirmed by pathology underwent MR SE -T1WI, FSE -T2WI scan and multi -phase dynamic enhanced scan before operation, the MR data were reviewed and analysed retrospectively in comparison with surgical and pathological results. Results 2, 1,3 and 3 of 9 lesions were located in neck, mediastinum, retroperitoneal and the adrenal gland respectively. The diameter of the tumours was 3.7 -16.4 cm,with a mean diameter of 7. 1 cm. The tumor presented homogeneous low signal intensity in 4 cases and inhomogeneous low signal intensity in 5 cases on SE T1WI, and presented homogeneous high signal intensity in 3 cases, inhomogeneous high signal intensity in 5 cases and homogeneous isosignal intensity in 1 cases on FSE T2WI, respectively. The lesions were slightly enhanced in artery phase and relatively obvious enhanced in portal venous and delayed phase. The enahnced patterns were whirlpool in 2 ,linear in 6 and spot in 1. The masses were well defined and showed a tendency of wedging into the space between adjacent organs and structures and encasing the large vessels, but without vessel compression or occlusion. Conclusion The dynamic MR features of ganglioneuromas are obvious, dynamic MR scans can improve the diagnostic rate of it.
