摘 要:目的观察潜艇人员血小板L-精氨酸(L-Arg)转运动力学,一氧化氮合酶(NOS)活性,血浆、血小板一氧化氮(NO)含量,探讨潜艇人员内皮功能变化,为预防心血管疾病提供依椐。方法48名健康男性潜艇人员及18名健康男性陆勤人员为对照者,静脉血制备血小板血浆,用放射性同位素标记法测定^3H-L-Arg在血小板转运中的动力学特征;测定血小板NO产物——亚硝酸盐量及NOS活性。结果潜艇艇员血小板L-Arg转运功能明显减低,L-Arg最大转运速度(Vmax)较同期陆勤人员低31.4%(P〈0,01),米氏常数(Km)值增加11.4%(P〈0.01),NOS活性较陆勤组减少51.1%(P〈0.01),血浆及血小板NO2^-水平分别低于陆勤组35.2%(P〈0.01)、82.8%(P〈0.01)。结论由于特殊环境,潜艇艇员存在血小板L-Arg/NOS/NO系统功能异常,L-Arg转运障碍,NOS活性显著减低,血浆及血小板NO生成明显减少,提示潜艇艇员有血管内皮功能受损征象,其特殊职业可影响心血管疾病发病学基础。Objective To observe conveying dynamics of L-arginine (L-Arg), activity of NO synthase (NOS) and contents of blood plasma, platelet and NO, approach the endothelium metergasis in sailors of submarine and provide some prophylaxises for cardiovascular diseases. Methods Forty-eight health sailors in submarine( SSM )group and eighteen in match health ground personnel (control group) served as the subjects. Isotope labelling method was used. ^3H-L-Arg transport, NO production and NOS activity in platelets were measured. Results Compared with the control group, the ability of L-Arg transport in platelets was lower; maximum transport velocity (Vmax) ,31.4% lower (P 〈0.01 ) ; michaelis comstant (Km) , 11.4% higher (P 〈 0.01 ) ; the activity of NOS in platelates, 51.1% lower (P 〈 0.01 ) and the productions of NO in blood plasma and platelets were obviously lower than those in SSM group. Conclusions An abnormal function in L-Arg/NOS/NO pathway was found in sailors of submarine because of special environment. Conveying disturbance of L-Arg, decrease of NOS activity, blood plasma and platelet NO can indicate that there are some fuction impaired signs of blood vessel endothelium in sailors of submarine. At the same time, special vocation may have some influences of pathogenesis base on cardiovascular diseases.
