外翻拖出式肛门外切除吻合术在女性中低位直肠癌中的应用

摘　　要：目的评价外翻拖出式肛门外切除吻合术在女性中低位直肠癌中应用的安全性和临床效果。方法回顾性分析我院1991年10月～2001年12月收治的56例女性中低位直肠癌患者，均行外翻拖出式肛门外切除吻合术：充分游离后切断结肠，将远端肠管连同肿瘤外翻于肛门外，边切除直肠肿瘤边行结肠直肠（肛管）吻合，术后还纳入肛门内。结果无手术死亡，无吻合漏发生，肛门控便能力良好。均获随访，平均68（28～124）月，术后5年肿瘤局部复发12．5％，5年生存率为73．2％。结论该法具有操作简单，安全可靠，并发症少的优点，对于女性中低位直肠癌患者尤为可行。Objective To evaluate the security and effect of pull-out resection and anastomosis for middle or low rectal cancer in female. Methods 56 female patients of middle or low rectal cancer underwent pull-out resection and anastomosis was reviewed. Results There were no operative mortality and anastomotic leakage, and the defecation continence was recovered. All cases were followed up for 28 to 124 months, 5- year local recurrence rate and survival rate was 12.5% and 73.2% respectively. Conclusion The procedure of pull-out resection and anastomosis is simple, safe, reliable, and prefer to treat female patients with middle or low rectal cancer with less complication.
