摘 要:目的探讨喉癌淋巴管生成的特点及其与喉癌淋巴结转移的关系。方法采用淋巴管内皮标记物D2-40进行SP法染色检测喉癌微淋巴管密度(microlymphatic density,MLD),分析其与临床病理特征的关系,并采用D2-40与Ki-67进行免疫组化双标法染色观察喉癌淋巴管内皮细胞的增殖活性。结果喉癌中有淋巴管形成,不同区域具有异质性,肿瘤周围的MLD为(13.09±4.62)个/视野,较肿瘤内部(6.51±3.14)个/视野及癌旁组织(4.27±2.76)个/视野显著升高,多呈扩张状态,其淋巴管内皮Ki-67指数(0.16±0.059)较其他区域显著升高,与喉癌淋巴结转移密切相关。结论喉癌中有新生淋巴管形成,主要位于肿瘤周围,癌周MLD增加与喉癌淋巴结转移密切相关,测定喉癌周围的MLD对判定是否发生淋巴结转移具有重要意义。OBJECTIVE To study the lymphangiogenesis in laryngeal carcinoma, and its relationship with lymph node metastasis. METHODS The microlymphatic density(MLD) was evaluated by immunohistochemistry using monoclonal antibody for D2-40, and its relationship with clinical and pathologic features was analyzed. The proliferative activity of lymphatic vessels was detected by double-labeling immunohistochemistry using Ki-67 and D2-40 antibodies. RESULTS The lymphatic vessels at the tumor borders often had large and open cavity, while those in the tumor had a reticular architecture with tiny and ill-defined cavity. The MLD at tumor borders(13.09±4.62) was significantly higher than that in the tumor (6.51±3.14) and in normal laryngeal mucosa (4.27±2.76) . Its Ki-67 labeling index (0.16 ± 0.059) was significantly higher than that in other portions. The MLD significantly correlated with regional lymph node metastasis. CONCLUSION Neolymphatic vessels are seen in laryngeal carcinoma, mainly at tumor borders. The high MLD at tumor borders is associated with lymph node metastasis. Evaluation of MLD at tumor borders may be useful in predicting lymph node metastasis in patients with laryngeal carcinoma.
