摘  要:目的探讨CT岩骨薄层扫描在乙状窦前手术入路中的应用价值。方法对10例成人头颅标本行高分辨率CT岩骨薄层扫描后,用磨钻对标本骨迷路轮廓化,分别测量与乙状窦前入路有关的后半规管及乙状窦沟与周围骨性标志的距离。结果CT扫描测量乙状窦沟宽是(11.44±1.79)mm,深是(5.27±1.93)mm,乙状窦沟底到乳突外表面的距离是(10.38±3.90)mm,乙状窦沟前壁到外耳道后壁的距离是(13.66±2.18)mm。后半规管最外侧至乳突外表面的最近距离是(13.44±1.86)mm,至乙状窦沟前缘的距离是(9.65±1.76)mm,其最后部至岩骨后壁的距离是(2.92±0.98)mm。对应的解剖测量结果分别是(11.26±1.58)mm、(5.12±1.88)mm、(10.26±3.78)mm(、13.74±1.96)mm、(13.86±1.98)mm(、9.82±1.91)mm和(3.12±1.08)mm。CT扫描测量与解剖测量结果无显著性差异(P>0.05)。结论CT扫描测量结果可代表实际的相关解剖结构的距离,CT岩骨薄层扫描可指导经岩乙状窦前入路中岩骨后外侧壁的安全磨除。Objective To evaluate the value of detail petrous bone CT scan for pre-sigmoid sinus operative approach. Methods The petrous bones of 10 adult Skull samples were scanned using detailed CT scanning (0.65 mm/scan). The labyrinths of the petrous bones were anatomically analyzed subsequently. The distances of the posterior semicircular canal and the groove of sigmoid sinus to the adjacent structures were measured. Results A. CT scan: width of groove of sigmoid sinus (GSS),Depth of GSS,the distance from the lateral wall of GSS to the surface of mastoid process, the distance from the anterior wall of GSS to the posterior wall of external auditory meatus,the distance from the most lateral portion of posterior semicircular canal (PSC) to the surface of mastoid process,the distance from the most lateral portion of PSC to the anterior edge of GSS, the distance from the most posterior portion of PSC to the posterior pyramidal wall was (11.44±1.79) mm, (5. 27 ± 1.93) mm, (10.38± 3.90) mm, (13.66± 2.18) mm, (13.44 ± 1.8) mm, (9.65 ± 1.76) mm, (2.92 ± 0.98) mm respectively, B. Anatomic measurement: the corresponding data was (11.26±1.58)mm,(5.12± 1.88)mm,(10.26±3.78) mm,(13.74±1.96)mm, (13.86±1.98)mm, (9.82±1.91)mm, (3.12±1.08)mm. There were no significant differences between the data obtained from CT scan and the data from anatomic measurement( P 〉0.05). Conclusion The data of petrous bone structures from CT scan are highly coincident with the data from anatomic measurement. Therefore,the detailed petrous bone CT scan is a reliable reference to direct the operation through pre-sigmoid sinus approach.
