Relationship between Peritumor Lymphatic Microvessel Density and Clinicopathology in Breast Invasive Ductual Carcinoma

摘  要:[目的]研究乳腺浸润性导管癌癌周淋巴管密度(peritumor lymphatic microvessel density,LMVDpt)与临床病理学特征及预后的关系。[方法]选取70例乳腺浸润性导管癌组织和12例乳腺纤维腺瘤组织,用免疫组化二步法以单克隆抗体D2-40标记淋巴管内皮细胞,光镜下计数癌周微淋巴管密度。[结果]①乳腺浸润性导管癌LMVDpt为13.17±2.89,明显高于乳腺纤维腺瘤组织(4.21±2.76),两者差异有显著性(P=0.000)。②LMVDpt与年龄、组织学分级,绝经状态以及雌激素受体状况之间无统计学联系(P值均>0.05),与肿瘤最大直径、腋窝淋巴结转移数目和pTNM分期之间有统计学关系(P值均<0.05)。③高LMVDpt组的5年无瘤存活率(53.1%)明显低于低LMVDpt组(73.9%)。[结论]LMVDpt在乳腺浸润性导管癌的扩散转移中起重要作用,可作为选择乳腺浸润性导管癌治疗方式及预测其淋巴结转移及预后的重要指标。[Purpose] To investigate the relationship between peritumor lymphatic microvessel density (LMVDpt) and clinico pathological features and prognosis in patients with breast invasive ductual carcinoma. [Methods] The data of 70 cases with breast invasive ductual carcinoma and 12 cases with breast fibroadenoma were enrolled.Monocolonal antibody D2-40 was used to mark endotheliocyte of lymphatic vessels by two-step immunohistochemical staining. Lymphatic mi- crovessels density was observed and counted under microscope. [Results] ①LMVDpt in breast invasive ductual carcinoma was 13.17±2.89, which was significantly higher than that in breast fi- broadenoma (4.21±2.76) (P:O.000). ②The LMVDpt wasn't related to age, histological degree, climacteric status and ER status(all P value〈0.05), but was closely correlated with longest diameter of tumor, number of axiilary positive lymph node invaded and pTNM stage(all P value〈0.05). ③The 5-year disease-free-survival (5y-DFS)in patients with high LMVDpt (53.1%) was significant- ly lower than that with low LMVDpt(73.9%). IConclusionl LMVDpt may play an important role in metastasis, and might be setve(l as a tumour marker to guide the therapy and evaluate the progno- sis for patients with breast invasive ductual carcinoma .
