摘 要:目的:检测核因子-κB受体活化因子配体(receptor activator of nuclear factor κB ligand,RANKL)和骨保护素(osteoprotegerin,OPG)这两个调控骨吸收的关键因子,在人类根尖肉芽肿(periapical granuloma,PG)和根尖囊肿(radicular cyst,RC)病损组织中的表达。方法:选择小鼠单克隆抗体为一抗,采用免疫组织化学Envison法检测20例根尖肉芽肿和20例根尖囊肿组织中RANKL和OPG的表达。选择6例因正畸需要拔除的健康牙齿的牙周膜组织作为对照。结果:在根尖肉芽肿和根尖囊肿组织中的RANKL蛋白表达均明显强于对照组中的RANKL表达[(43.74±8.40)vs(15.47±2.59),(40.33±7.53)vs(15.47±2.59),均P=0.000],但根尖肉芽肿和根尖囊肿组织之间的RANKL表达差异无统计学意义。在根尖肉芽肿、根尖囊肿和对照组中的OPG蛋白表达水平分别为27.81±5.17,26.35±3.86和24.33±3.50,3组间的OPG表达差异无统计学意义。根尖肉芽肿和根尖囊肿组织中的RANKL/OPG比率与对照组相比,均有显著上升[(1.59±0.26)vs(0.64±0.10),(1.54±0.24)vs(0.64±0.10),均P=0.000],但RANKL/OPG的比率在根尖肉芽肿和根尖囊肿两组间的差异无统计学意义。结论:RNAKL和OPG均能表达于慢性根尖周炎病损组织中。RANKL可能与慢性根尖周炎骨吸收活动关系密切,同时RANKL和OPG可能在根尖周病的疾病进展中发挥了重要作用。Objective:To investigate the expression of key mediators that regulate bone resorption, receptor activator of nuclear factor-KB ligand (RANKL) and osteoprotegerin (OPG) in human perapical granuloma (PG) and radieular cyst (RC) characterized by periapieal bone destruction. Methods: Immunohistoehemieal technique was performed using monoelonal antibody to detect RANKL and OPG expres- sion in PG (n = 20 ) and RC (n = 20 ). As control group, healthy periodontal ligament tissues (n = 6 ) were obtained from teeth with orthodontic indication of extraction. Results: The RANKL protein expression was significantly higher in PG (43.74 ±8.40) and RC (40.33 ±7.53) than in control group (15.47 ±2.59, P = 0.000) , but no statistical significance could be found between PG and RC groups (P = 0. 161 ) . The expression of OPG in PG (27.81±5.17), RC (26.35 ±3.86) and control group (24.33 ± 3.50) show no statistical significance ( P 〉 0.05 ). Moreover, RANKL/OPG ratio in PG (1.59 ±0.26) and RC (1.54 ±0.24) was much higher than that in control group (0.64 ±0.10, P = 0.000) , however the ratio of RANKL/OPG showed no significant difference between PG and RC groups (P = 0.504) . Conclusion:RANKL and OPG were observed in the periapieal lesions. RANKL may be responsible for bone resorption in periapieal lesions. RANKL and OPG may play an important role in the development of periapieal lesions.
