摘  要:目的评价超声引导穿刺活检对乳腺癌腋窝淋巴结定性诊断的价值。方法以腋淋巴结清扫术后病检结果为标准,对腋窝淋巴结肿大的乳腺癌病例72例经超声引导穿刺病理学诊断结果进行分析。活检取出的组织以能作出明确的组织病理学诊断的为取材满意。结果本组穿刺72例,其中67例结果与术后病理相符,诊断敏感性95.0%,特异性100%,准确性95.71%。假阴性3例,无假阳性病例,2例取材样本不足(大部为血凝块)。未出现任何与穿刺相关明显并发症。结论超声引导下对乳腺癌腋淋巴结穿刺技术具有定位准确,可靠性高,创伤小,操作简单等优点,对乳腺癌的病情预后估计及治疗方案的选择有重要的应用价值。Objective To study the value of ultrasound - guided needle hiopsy for the qualitation of axillary lymphadenectasis in breast cancer. Methods The technique of ultrasound guided needle biopsy was carried out in 72 patients with breast cancer and axillary lymphadenectasis. The patients were accepted subsequent operation and pathological examinations. A biopsy was considered successful if a specific benign or malignant diagnosis was rendered by the pathologist, Results Seventy ultrasound guided needle biopsy samples were used in this study, Compared with the pathological results of excisional axillary nodes during surgery, the sensitivity, specificity, accuracy of ultrasound guided needle biopsy were 95. 0%, 100% and 95. 71% respectively; 3 false negative and zero false positive were found, Two samples were not used in this study because of inadequate specimen. None of the patients had complications related to ultrasound guided puncture procedures, Conclusion Ultrasound guided needle biopsy for the qualitation of axillary lymphadenectasis has the advantages of precise orientation, high reliability, small injury and simple operation, It is valuable for the prediction of malignant degree and helpful for the selection of treatment methods in breast carcinoma.
