不同剂量善宁治疗食道胃底静脉曲张破裂出血的疗效观察

摘　　要：目的观察不同剂量善宁治疗食道胃底静脉曲张破裂出血的疗效。方法将食道胃底静脉曲张破裂出血的患者467例随机分成3组,分别为大剂量善宁组(A组),常规剂量善宁组(B组),小剂量善宁组(C组)。3组均首先给予善宁0.1mg静脉注射,之后分别以不同剂量善宁持续静脉滴注:A组:0.05 mg/h;B组:0.025 mg/h;C组:0.015 mg/h。观察3组的止血疗效和停药后7天内的再出血率。另外大剂量善宁组患者出血停止后分为三组,分别给予善宁0.05 mg/h、0.025 mg/h、0.015 mg/h维持静点48小时,再观察疗效和停药后的再出血率。结果A、B、C 3组6小时有效率分别为53.8%,30.6%和30.6%;12小时有效率分别为93.1%,72.2%和69.5%;24小时有效率分别为94.9%,75%和73.9%;总有效率分别为96.8%,75%和73.9%(PA、B<0.05),(PB、C>0.05)。结论加倍剂量善宁对食道胃底静脉曲张患者的止血效果要优于常规剂量,止血成功后可以应用常规剂量或小剂量善宁继续维持治疗。小剂量善宁对于食道胃底静脉曲张的患者止血效果与常规剂量无明显差异。Objective To investigate the efficacy of different dosage of somatostain on treatment of esophageal variceal bleeding. Methods 467 cases were randomly divided into three groups as A,B, C; patients in three groups received an intravenous injection of 0. ling somatosatin, then at a constant speed of 0. 05 mg/h, 0. 025 mg/h, 0. 015 mg/h for 48 hours. Hematischesis was observed at 6 hours, 12 hours, 24 hours, and 48 hours. Results 6 hours hematischesis rate were respectively 46. 6% , 30. 6% and 30. 4%. 48 hours hematischesis rate were 96. 5%, 89% and 86. 5% （ PA、B 〈 0. 05 ） ,PB、C 〉 0. 05）. Conclusion Double dosage of somatostatin has better effect than the convention dosage and small dosage has no difference with the convention dosage on the treatment of acute bleeding of esophageal varieeal.
