摘  要:目的:研究正中神经短潜伏期体感诱发电位(MN-SSEP)与中潜伏期体感诱发电位(MN-MSEP)在脑梗死(CI)患者应用中各自的特点及其优势。方法:对37例CI患者与21位正常对照者进行MN-SSEP和MN-MSEP检测和比较。结果:①CI组中患侧MN-SSEP中N20波消失和MN- MSEP各波消失;②CI组中患侧MN-SSEP正常,MN-MSEP中N20、P25的峰潜伏期(PL)正常,而N35、P45、N60的PL延长,这些异常成分与健侧比较,差异有显著意义(P<0.01),以N60的PL延长(异常)尤为显著;③CI组的健侧与对照组比较,差异无显著意义(P>0.05)。结论:N20波的消失与皮层功能缺失或上传感觉通路阻断有关。N20波正常,而其后的皮层电位异常可能与皮层间的网络连接被破坏使传导受阻有关,N60波异常的突出表现可以作为观察皮层功能变化的敏感指标。Objective: To compare the features and applicafion advantages of median nerve short la tency SEP(MN SSEP) with middle latency SEP(MN-MSEP) in patients with cerebral infarction. Methods: Both MN SSEP and MN MSEP detection were performed in 37 patients with cerebral infarction and 21 normal controls. Results: First of all, paralyzed lateral, the cortical components (N20, P25, N35, P45, N60) of MN-SSEP and MN-MSEP disappeared, whereas the subcortical components(Ng, N13) of MN-SSEP were normal in the infracted lateral; Second, the all components of MN SSEP and N20, P25 components of MN MSEP were normal in the infracted lateral, whereas the peak latency(PL)of N35, P45, N60 components of MN-MSEP was elongated, and had significant difference as compared with those of contralateral(P〈0.01). Thirdly, the MN-SSEP and the MN-MSEP in the lateral of noninfarc tion had no significant difference compared with those of normal controls. Conclusion: The disappearance of N20 waves is related to cortical function deficit or the blocked sensory gateway, while the abnormity rate of those components behind N20 could be related to the dysfunction between the cortical networks. The abnormity of N60 component indicates the sensitivity indication of cortical dysfunction.
