腹腔镜下子宫广泛切除术后腹膜阴道延长术临床疗效观察

摘　　要：目的探讨腹腔镜下子宫广泛切除术后腹膜阴道延长术的临床疗效。方法9例子宫颈癌患者行腹腔镜下子宫广泛切除加盆腔淋巴结切除,同时将盆腔腹膜与残留阴道壁吻合,盆腹膜顶端封闭,形成延长的阴道腔隙。术后观察随访3～15个月,观察有无增加腹腔镜下子宫广泛切除术并发症和阴道长度变化及患者术后性生活情况。结果9例患者经过随访未增加腹腔镜下子宫广泛切除术并发症,9例患者阴道长10～12cm,无粘连、狭窄,术后性生活恢复时间(61±5.5)d,均能完成正常性生活。结论腹腔镜下子宫广泛切除术后腹膜阴道延长术能恢复患者阴道生理解剖结构,有效改善患者术后性生活质量而不增加手术并发症,是一种安全有效的手术方式。[Purpose] To inquire the clinic effect of the peritoneum colpoplasty and lengthening after uterus extensive resection by laparoscope,[Methods] In nine cases of the cervix carcinoma, uterus extensive resection was done and pelvic kidney lymph nodes were resected by laparoscope, at the same time the pelvic peritoneum and remaining vaginal parietal were anastomosed, the top of the pelvic peritoneum was blocked, the elongation vaginal canal was formed. Post-operation follow-up persisted for three to fifteen months, the complication of uterus extensive resection and the post-operation sex life condition and the transformation vaginal canal length were observed. [ Resuits] Nine cases were followed up, there was no increase in the complication of uterus extensive resection by laparoscope, the vaginal canal length was ten to twelve centimeters, no adherence and constriction appeared. All the cases can have normal sex life and the post-operation recovery time of the sex life about two months. [Conclusion] Peritoneum colpoplasty and lengthening after uterus extensive resection by laparoscope is an effective and safe operation, it can restore the physiological anatomic structure and improve the quality of the sex life without increasing complications.
