摘  要:目的:探讨功能性胰腺内分泌肿瘤与周围正常胰腺组织的MSCT灌注成像特征。材料和方法:12例功能性胰腺内分泌肿瘤患者,男性5例,女性7例,年龄25~69岁,平均43.4岁。采用GELight Speed16层MSCT进行灌注扫描,以3.5m/s的速度团注对比剂50ml,延迟5~7s,对选定胰腺病变层面进行同层动态增强检查扫描。将获得的原始图像传到GEAW4.2工作站,用CT-Perfusion3软件进行分析测量胰腺病变及周围正常胰腺组织的时间密度曲线(TDC)、血流量(BF)、血容量(BV)、平均通过时间(MTT)、表面通透性(PS)及达峰时间(TTP)。结果:功能性胰腺内分泌肿瘤的BF、BV、MTT及PS分别为(287.73±140.97)ml/(min·100g)、(18.48±5.17)ml/100g、(5.96±3.07)s及(17.37±11.10)ml/(min·100g)。周围正常胰腺组织的BF、BV、MTT及PS分别为(162.47±92.44)ml/(min·100g)、(11.77±3.54)ml/100g、(7.13±3.02)s及(17.54±12.88)ml/(min·100g)。功能性胰腺内分泌肿瘤组织BF和BV高于周围正常胰腺组织(P=0.02,t=3.257;P=0.000,t=4.71),MTT及PS无统计学意义(P=0.230,t=-1.218;P=0.965,t=-0.044)。功能性胰腺内分泌肿瘤组织及周围正常胰腺组织TTP分别(27.17±4.98)s及(27.96±4.09)s,两者之间无统计学意义(P=0.591,t=-0.542)。结论:功能性胰腺内分泌肿瘤组织与周围正常胰腺组织间存在灌注上的差异,对其诊断具有重要的提示意义。Purpose: To evaluate the differentia of the characteristics of MSCT perfusion between functioning pancreatic endocrine tumor and tumor - free pancreatic parenchyma. Materials and Methods: 12 patients(5 male, 7 female, age range 25- 69y, mean age 43.4y)who suffering from functioning pancreatic endocrine tumor underwent CT perfusion scan by GE LightSpeed 16 MSCT. Dynamic enhancement scanning was carried out for the same chosen sections of pancreatic lesions after contrast agent bolus injection with scan delay time 5 - 7s, injection rate 3.5ml/s and total injection volume 50ml. Then the raw images were delivered to GE AW4.2 Workstation and the time density curves(TDC), blood flow(BF), blood volume (BV), mean transit time(MTT), vascular permeability - surface (PS) and time to peak(TTP) of pancreatic lesions and tumor - free pancreas were calculated and analyzed with the dedicated CT - perfusion 3 software. Results: The mean BF, BV, MTT and PS of the functioning pancreatic endocrine tumor were (287.73±140.97)ml/(min·100g), (18.48±5.17)ml/100g, (5.96±3.07)s, (17.37±11.10)ml/ (min·100g), respectively. The mean BF, BV, MTT and PS of normal pancreas tissue were (162.47 ±92.44) ml/(min · 100g), ( 11.77±3.54) ml/100g, (7.13±3.02) s, (17.54±12.88) ml/(min · 100g), respectively. Functioning pancreatic endocrine tumor had higher BF and BV(P = 0.02, t = 3. 257; P = 0. 000, t = 4.71) but no significant difference in MTT and PS (P = 0. 230, t = - 1. 218; P = 0.965, t = - 0.044) compared with those of normal pancreas.
