摘  要:目的探讨肝细胞癌(HCC)超声造影(CEUS)灌注参数与微血管的相关性。方法47例HCC行术前CEUS,其中43例行脱机时间-强度曲线(time-intensity curve,TIC)软件分析。术后病理标本行病理分级和CD34免疫组化染色,并与灌注参数进行对比、分析。结果微血管密度(microvessel density,MVD)在47例不同病理分级的HCC间有显著性差异(P<0.05),但MVD在不同大小的HCC间无显著性差异(P>0.05);47例不同病理分级HCC的微血管形态有显著性差别(P<0.05);43例HCC的MVD值与病灶内峰值强度(PI)、效应持续时间成正相关(相关系数分别为r=0.36,P=0.02;r=0.45,P=0.003),与增强斜率和增强时间无关(P>0.05);43例不同微血管分型HCC间灌注参数比较均无统计学差异(P>0.05)。结论HCC的实时超声造影表现受肿瘤微血管生成的影响,HCC的灌注参数与肿瘤微血管具有较高的相关性,CEUS可以对活体内肿瘤组织的微血管生成情况进行较准确的判断。Objective To explore the correlation of contrast-enhanced ultrasonography (CEUS) with tumor angiogenesis in hepatocellular carcinoma (HCC). Methods CEUS was performed before operation in 47 patients with HCC. Time intensity curves (TIC) in 43 cases with HCC were analyzed. Pathologic grading for HCC was determined according to the scale of Edmonson. Tissue specimens of HCC in 47 cases were stained with CD34 by immunohistochemical method. The perfusion parameters obtained by the TIC were analysed to explore the relationship between the perfusion data and angiogenesis. Results There were significant differences between microvessel density (MVD) and pathologic grading of HCC (P〈0.05), but not between tumor size and pathologic grading of HCC (P〉0.05) ; The characteristics of microvessels were significantly related to pathologic grading in HCC (P〈0.05) ; There were statistically positive correlations between peak intensity (PI) and enhancement duration time (EDT) and MVD (r= 0.36, P= 0.02; r= 0. 45, P= 0. 003), respectively. There were no obvious correlation between the perfusion parameters and the patterns of microvessels in 43 HCC(P〉0.05). Conclusions The perfusion data correlated with tumor angiogenesis in HCC, thus CEUS offers a new way to judge the tumor angiogenesis in vivo.
