草酸铂联合氟尿嘧啶和亚叶酸钙治疗晚期大肠癌36例临床观察

摘　　要：目的观察草酸铂(L-OHP)联合氟尿嘧啶(5-Fu)和亚叶酸钙(CF)治疗晚期大肠癌的近期疗效和毒副反应。方法经病理组织学诊断的晚期大肠癌36例,用L-OHP 130 mg/m^2,静脉滴注4 h,每天1次;CF 200 mg,静脉滴注2 d,继以5-Fu 400 mg/m^2,d1,d2,静脉滴入,5-Fu 600 mg/m^2,微量泵泵入48 h以上,。每3周重复1次,作为1个治疗周期,治疗2个周期后评定疗效。结果36例总有效率为13例(37.1%),完全缓解1例(2.8%),部分缓解12例(33.3%),无变化14例(38.9%),进展9例(25%)。主要的毒副作用为神经毒性、骨髓抑制、恶心呕吐等,无治疗相关性死亡患者。结论L-OHP联合大剂量CF及5-Fu治疗晚期大肠癌疗效较好,毒副反应能为患者耐受,值得临床推广应用。Objective To study the recently curative, noxious and side effect of drug combination of oxaliplatin （ L-OHP）, fluorouracil （5-Fu） and calcium folinate（CF） in the cure of colorectal cancer in advanced stage. Methods 36 cases of colorectal cancer in advanced stage diagnosed by pathology was involved. L-OHP 130 mg/m^2 was injected in 4 hours in dl, CF 200 mg was inject in first two days, followed by the injection of 5 - Fu 400 mg/m^2 in d1,2, and the micro pumping of 5 - Fu 600 mg/m2 over 48 hours in d2. The treatment was repeated every three weeks as one cycle and the curative effect was evaluated after 2 cycles. Results The total effective rate of the 36 cases was 37.1% （13cases） , the complete remission（CR） was lcase（2.8% ）, the partial remission was 12cases（33.3% ）, no changed was 14 cases （38.9%） and progression was 9 cases （25 % ）. The main noxious and side effects were neurotoxicity, myelosuppression, nausea and vomiting, etc. There was no cure related death. Conclusion The curative effect of drug combination of oxaliplatin （ L-OHP）, fluorouracil （5-Fu） and calcium folinate （CF） in the cure of colorectal cancer was good and the noxious and side effect was tolerable, meanwhile the combination was worth to be used widely.
