摘  要:目的探讨磁共振弥散加权、灌注及波谱成像在髄母细胞瘤诊断中的应用价值。方法回顾性分析11例经手术病理证实的髄母细胞瘤常规MRI及弥散、灌注、波谱检查资料。结果测得肿瘤实性部分的ADC值明显降低,11例均低于1.0×10-3mm2/sec;灌注增强造影剂平均通过时间延长,相对值范围为1.031-1.087,时间-信号曲线显示造影剂流出段平缓;波谱曲线所有病例胆碱(Cho)峰均明显增高,8例于3.4ppm处可见氨基乙磺酸峰(Tau),呈峰值略低的M型。结论弥散加权、灌注及波谱成像联合应用,并与常规MRI结合有助提高髓母细胞瘤的诊断准确率,具有重要临床意义。Objective To evaluate the value of diffusion - weighted imaging, perfusion and proton MR spectroscopy in the diagnosis of medulloblastoma. Methods The conventional MR images, diffusion, perfusion and proton MR spectroscopy information were retrospectively analyzed of 11 cases medulloblastoma patients who were all underwent operation and then got the apparent diffusion coefficient, mean time to enhance, time intensity curve and MRS curve of tumor. Results The apparent diffusion coefficient values of the mass all were decreased, less than 1.0 × 10^ -3; the mean time to enhance was prolonged with the extent about 1. 031 - 1. 087, the exuded stage of the time intensity curve was slow; all the patients showed elevated choline peaks, Taurine which like an M was detected at approximately 3.4 ppm in 8 of all, Conclusion Combination with diffusion - weighted imaging, perfusion and proton MR spectroscopy on the basis of conventional MR is useful in the differentiation of medulloblastoma from other tumors.
