睾丸原发性非霍奇金淋巴瘤病理特征分析

摘　　要：目的探讨睾丸原发性非霍奇金淋巴瘤的临床表现、病理形态学特征、免疫表型、诊断及鉴别诊断。方法对6例睾丸原发性淋巴瘤进行形态学和免疫组化染色观察,结合文献对其临床病理特点进行分析。结果患者年龄62-75岁。主要症状为睾丸无痛性肿大。经免疫组化证实6例均为非霍奇金弥漫性大B细胞性淋巴瘤。其中4例为中心母细胞为主型,1例为中心母细胞-中心细胞混合型,1例为B免疫母细胞型。免疫组化染色肿瘤细胞CD20、CD45RO、CD79a均阳性,CD3、CD45Ra、CD30、PLAP均阴性。结论睾丸原发性恶性淋巴瘤中老年人多见,出现睾丸弥漫性肿大,以非霍奇金弥漫性大B细胞性淋巴瘤为主,正确的病理诊断对指导临床治疗和判断预后有重要的作用。Objective To investigate the clinical manifestation, pathological features, immunohistochemical, diagnosis and differential diagnosis in the testicular primary malignant non - Hodgkin＇s lymphoma. Methods The morphological and immunohistochemistry. of testis in six cases with primary lymphoma were observed and literature and the clinical features were analyzed. Results Patients＇s age was from 62 -75 years old and the main symptom was of painless testicular enlargement. Six cases were non - Hodgkin ＇s diffuse large B - cell lymphoma by immunohistochemical staining. 4 cases of them were centroblastic lymphoma, one case was mixed of immunoblastic lymphoma and another one was of B - cell lymphoma. Immunohistochemical staining for CD20, CD45 RO and CD79 a were positive; and CD3 , CD45 Ra, CD30, PLAP were negative. Conclusion Primary lymphoma of the testis is mostly in middle - aged and the elder. Most cases are non - Hodgking diffuse large B - cell lymphoma in these diffuse swelling of testicles. Precise pathological diagnosis can play important role for clinical treatment and prognosis.
