摘  要:目的:探讨常规脑电图(electroencephalogram,EEG)、视频脑电图(video electroencephalogram,VEEG)、单光子发射计算机断层扫描(single photon emission computed tomography,SPECT)、头颅CT(computerized tomography)及MRI(magnetic resonance imaging)检查在癫痫诊断中的价值。方法:对126例已确诊的癫痫病人分别进行EEG、VEEG、SPECT、CT及MRI检查,对EEG、VEEG、SPECT、CT、MRI定位致痫灶的情况进行对比研究。结果:EEG异常126例,局灶棘波52例(41.3%);VEEG异常126例,局灶棘波114例(90.5%),两组局灶棘波检出率有显著性差异(P<0.01)。SPECT异常105例(83.3%),MRI异常32例(占55例的58.2%),CT异常5例(占48例的10.4%)。结论:VEEG、EEG、SPECT、MRI检查结合临床表现,对难治性癫痫病人术前致痫灶的定位和指导手术治疗有较大的应用价值。Objective:To evaluate the value of imaging examination of electroencephalogram (EEG), video electroencephalogram (VEEG), single photon emission computed topography (SPECT), CT and MIR in diagnosis of epilepsy. Methods:126 patients with epilepsy were examined by EEG、VEEG、SPECT and MRI, respectively, and localization of epileptogenic focus were identified and analyzed among the different examined groups. Results:All cases showed abnormal EEG and 52 cases showed local spike waves. All cases also showed abnormal VEEG, but 114 eases showed local spike waves, Therefore, EEG and VEEG had significantly different detection rates (P〈0. 01). 105 abnormal SPECT cases were found among 126(83. 3%); 32 abnormal MR/cases were found among 55 cases (58. 2%); and 5 abnormal CT cases found among 48 cases(10. 4%). Conclusions:VEEG、EEG、SPECT and MRI should be used jointly and the localization of epileptogenic focus of patients with intractable epilepsy should be combinedl with their clinical manifestations besides of above examination.
