摘　　要：目的探讨以眩晕为主要表现的原发性低颅压综合征（SIH）的临床发病特点与治疗方法。方法分析21例非头痛性SIH患者的临床资料，均统一使用补液、氨茶碱、参芪扶正液等治疗。结果21例患者急性起病12例。缓慢性发病9例。多数（14例1有持续紧张工作史。其主要症状为体位性头晕：神经体征少见，部分有轻度颈项抵抗；影像学和脑脊液可有轻度异常。使用推荐方法治疗的疗效肯定、迅速。结论SIH可以表现为体位性眩晕。与持续快节奏、高压力工作有关，经积极治疗后预后好。Objective To explore the clinical features and therapeutic approaches of spontaneous intraeranial hypotension （SIH） dominated by the major manifestation of vertigo, Methods Clinical data of 21 non-headache SIH patients with vertigo as the major performance were analyzed. Aminophylline, Shenqi Fuzheng injection and adequate fluid infusion were used for treatment on all these cases. Results Among the 21 cases, 12 had an acute onset and 9 had a chronic onset, Most （14 cases） had a sustained intensive working history, Postural vertigo without positive neurological signs was the major symptom, Some cases had slight neck resistance. Mild abnormalities could be found in imaging and the cerebrospinal fluid examination, Treatment by recommended therapies was effective and rapid. Conclusion Postural vertigo without positive neurological signs can dominate the major symptom of SIH, which is related with sustained, rapid and intensive work and has favorable prognosis
