D2-40在原发性胃腺癌淋巴管浸润中的标记及临床病理意义

摘　　要：目的探讨一种新的淋巴管特异标记物D2-40在判断原发性胃腺癌淋巴管浸润中的作用及其临床病理意义。方法应用单克隆抗体D2-40检测74例原发性胃腺癌淋巴管浸润的情况,并分析其和临床病理参数之间及和癌周淋巴结微转移之间的关系。结果D2-40选择性地表达在CD31染色阴性的淋巴管内皮细胞。HE染色和D2-40染色判断肿瘤淋巴管浸润的阳性率分别是16.2%(12/74)和52.7%(39/74),两者间差异有统计学意义(P<0.001)。统计学相关分析显示借助D2-40(r=0.641)和HE(r=0.415)方法判断淋巴管浸润均相关于淋巴结微转移,但两者间差异有统计学意义(P<0.01),提示采用D2-40标记判断淋巴管浸润来预测淋巴结微转移是一种更可靠的手段。结论D2-40抗体的应用较HE染色方法更敏感和特异地检测出肿瘤的淋巴管浸润,并且在预测淋巴结微转移方面有更高的可靠性,因此具有良好的临床病理应用价值。Purpose To investigate the application of D2-40, a new specific marker of lymphatic endothelium, in evaluation of the lymphatic invasion of primary gastric adenocarcinoma and its clinicopathological significance. Methods Monoclonal antibody D2-40 was used to detect the lymphatic invasion in 74 cases of primary gastric adenocarcinomas, and the relationships between lymphatic invasion and clinicopathologlcal findings or lymph node micrometastasis were analyzed. Results D2-40 showed specific staining for the lymphatic endothelium, which was negative for CD31. Positive rate of lymphatic invasion by HE staining was 16. 2% （12/74） and that by D2-40 was 52. 7% （39/74）. Significant difference was found between them. Statistical analysis disclosed that the lymphatic invasion detected by D2-40 （ r = 0. 641 ） and by HE staining （ r = 0. 415 ） were both correlated with lymph node micrometastasis （ P 〈 0. 01 ）. Lymph node micrometastasis was significantly related with lymphatic invasion, which were visualized by D2-40 immunohistoehemistry. Conclusions The detection of lymphatic invasion by D2-40 is more sensitive than HE staining, and the higher reliability of predicting lymph node micrometastasis by D2-40 is suggested. Therefore, it has applicable value in clinical pathology.
