内镜下射频消融联合药物治疗幽门螺杆菌阴性的成熟型疣状胃炎56例

摘　　要：目的:探讨内镜下射频消融联合西咪替丁和胃复春治疗幽门螺杆菌(H pylori)阴性的成熟型疣状胃炎的临床应用价值.方法:对符合标准确诊为H pylori阴性的成熟型疣状胃炎患者112例随机分为治疗组和对照组,治疗组先采取胃镜下射频消融治疗,将隆起病变灼平,然后服用西咪替丁800 mg/次,1次/d,晚上睡前服,连服30 d,胃复春4片,3次/d,饭前服,连服90 d,对照组不予射频消融治疗,用药同治疗组,两组均于疗程结束及随访1年后分别复查胃镜以观察治愈率,总有效率和复发率.结果:治疗组的治愈率,总有效率明显高于对照组(77.8%vs 45.5%,P<0.01;98.1%vs 83.7%,P<0.05);治疗组的1年复发率明显低于对照组(2.4%vs 48.0%,P<0.01),两组间有显著性差异.结论:内镜下射频消融联合西咪替丁和胃复春治疗H pylori阴性的成熟型疣状胃炎是一种有效,安全的方法.AIM： To discuss the clinical value of radiofrequency ablation （RFA） combined with cimetidine and Weifuchun for mature verrucosa gastritis without Helicobacter pylori infection. METHODS： A total of 112 patients with mature verrucosa gastritis without H pylori infection were randomly divided into treatment group and control group. The treatment group received RFA followed by cimetidine （800 mg, quaque nocte） for 30 d and Weifuchun （four pieces, three times daily, ante cibum） for 90 d. The control group received the same drugs without RFA. All patients were checked by gastroscopy immediately and 1 year after treatment, to observe the recovery, total effectiveness and relapse rate. RESULTS： The recovery and total effectiveness of the treatment group were significantly higher than those of the control group （77.8% vs 45,5%,P 〈 0.01; 98.1% vs 83.7%, P 〈 0.05）. The relapse rate after 1 year in the treatment group was significantly lower than that in the control group （2.4% vs 48.0%, P 〈 0.01）. There was a significant difference between the two groups. CONCLUSION： RFA combined with cimetidine and Weifuchun is safe and effective for the treatment of mature verrucosa gastritis without H pylori infection.
