中药联合化疗治疗非何杰金淋巴瘤112例临床观察

摘　　要：目的：为探求治疗非何杰金淋巴瘤（Ｎｏｎ－Ｈｏｄｇｋｉｎ′ｓＬｙｍｐｈｏｍａ；ＮＨＬ）的有效方法。方法：选择１６７例患者，随机分为两组，进行前瞻性对比治疗，治疗组１１２例用中药联合化疗，对照组５５例单用化疗。结果：治疗组有效率（ＣＲ＋ＰＲ）９１．９６％，１、３、５年生存率分别为８５．７％、５４．５％、２９．５％，中位生存期５５４天；对照组有效率及１、３、５年生存率分别为７２．７３％、７６．４％、３８．２％、１８．２％，中位生存期４６５天，两组有效率及３年生存率有显著性差异（Ｐ＜０．０５）。治疗组治后ＮＫ细胞、ＯＫＴ３、ＯＫＴ４、ＯＫＴ４／ＯＫＴ８比值均较治疗前明显提高（Ｐ＜０．０１），血小板粘附率、血沉及全血比粘度明显下降（Ｐ＜０．０１）；对照组治疗前后各值变化不明显。毒副反应治疗组较对照组明显少而轻。结论：中药有提高ＮＨＬ患者免疫功能及降低血液粘度的作用，中药联合化疗治疗ＮＨＬ安全有效，值得推广。To seek for the effective therapeutical method in treating non-Hodgkin's lymphoma (NHL). Methods: One hundred and sixty seven patients with non-Hodgkin's lymphoma were randomly divided into two groups, the treatment group, which consisted of 112 cases using Chinese herbs combined with chemotherapy and 55 cases of control group were treated by chemotherapy only. Results: The effective rate (CR+PR) in the combined group was 91. 96% and survival rates of 1-,3- and 5-year were 85. 7%, 54. 5% and 29. 5% respectively, and median survival time was 554 days. In control group the effective rate was 72. 73% and 1-, 3- and 5-year survival rates were 76. 4%, 38. 2% and 18. 2% respectively, and the median survival time was 465 days. The difference of effective rates or 3-year survival rates between two groups was significant (P<0. 05). In the combined group the activity of NK cell, OKT3, OKT4 and ratio of OKT4/OKT8 were obviously raised after treatment (P<0. 01 ). And the level of platelet adhesion rate and the blood viscosity markedly decreased (P<0.01), but in the control group the values of these indexes did not distinctly change. Conclusion: Chinese herbs could enhance the immunologic function and improve the viscosity of blood of the patients with NHL. The side effect in the combination therapy group was less and milder than that in the chemotherapy group. These showed that Chinese herbs combined with chemotherapy was a safe and effective method for treating NHL and deserve to be recommended.
