平阳霉素化疗与口腔鳞癌多药耐药基因表达的关系及其临床意义

摘  要:目的通过对口腔鳞癌组织(OSCC)化疗前后,多药耐药基因mdrl的表达进行检测,观察化疗药物平阳霉素对口腔鳞癌mdr1表达的影响。方法口腔鳞癌31例,术前诱导化疗。停药1周后手术治疗,切除肿物。化疗前后分别取标本。用RT—PCR法分别检测mdr1mRNA的表达。结合临床病理分级,对实验结果进行统计学分析。结果口腔正常粘膜存在原发性mdr1mRNA表达,阳性率22.2%(2/9)。化疗前高分化鳞癌mdrlmRNA表达阳性率38.9%(7/18),化疗后高分化鳞癌表达阳性率44.4%(8/18),二者相比差异无显著性(P〉0.05)(表1)。化疗前中低分化鳞癌mdr1mRNA表达阳性率30.8%(4/13),化疗后中低分化鳞癌表达阳性率38.5%(5/13),二者相比差异无显著性(P>0.05)(表2)。结论口腔正常粘膜部分存在原发性mdrl表达。平阳霉素单一化疗,可能不诱导口腔鳞癌组织mdr1表达的增高,即不会产生获得性耐药,平阳霉素对口腔鳞癌化疗有效。Objective To detect multidrug resistant gene (mdrl) in OSCC before and after chemotherapy and observe the effections of pingyang mycin to the expression of mdrl. Method 31 oral squamous cell carcinoma(OSC) patients were treated by pre-operative chemotherapy. After one week drug withdrawed, all patients had their tamor resect. Take their tumor samples before and after pingyangmycin chemotherapy. Result There is primary expression of mdrlmRNA in normal oral mucosa, and expression rate is 22.2%(2/9). There is no significant differences (P〉0.05) between before and after chemotherapy in well-differentiated OSCC. So it is in media and poorly differentiated OSCC. Conclusion Primary mdrl expression is in partal normal oral mucosa. Pingyangmycin single-chemotherapy may not induce high mdrl expression, so there will be no acquired resistance situation pingyagmycin, chemotherapy is useful for the treatment of OSC.
