摘 要:目的建立大鼠急性肺血栓栓塞症(PTE)模型,动态观察内皮素-1(ET-1),环氧合酶-2(COX-2)和黏附分子CD54在肺内的表达及血浆中前列腺素代谢产物血栓烷素B2(TXB2)和6-酮-前列腺素F1α(6-K-PGF1α)的变化,并探讨中药川芎嗪对它们的影响。方法采用自体血栓回输法建立动物模型,将大鼠随机分为空白对照组、PTE模型组和川芎嗪治疗组。采用常规病理检查,免疫组化法检测肺内ET-1,COX-2和CD54蛋白的表达水平,放射免疫法测定血浆中TXB2和6-K-PGF1α的浓度变化。结果病理检查显示PTE组肺组织炎症损伤明显,川芎嗪治疗组损伤减轻。免疫组化检测显示模型组7d后ET-1、COX-2和CD54蛋白表达水平较对照组明显增高,组间差异有统计学意义(均P<0.05),而川芎嗪治疗7d后与对照组组间差异无统计学意义(均P>0.05)。放射免疫检测显示模型组7d后TXB2与6-K-PGF1α血浆浓度之比(T/P)较对照组明显增高,组间差异有统计学意义(P<0.05),而川芎嗪治疗7d后与对照组组间差异无统计学意义(P>0.05)。结论川芎嗪治疗可明显减少急性肺血栓栓塞症大鼠ET-1、COX-2、CD54的表达,减轻炎症反应。提示在肺栓塞常规溶栓治疗时,可结合中药治疗,以取得较好效果。Objective To study the expression levels of endothelin-1 (ET-1), cyclooxygenase-2 (COX-2) and CD54 in the lung tissue, and the concentrations of thromboxane B2 (TXB2) and 6-keto-prostaglandin Flo (6-K-PGFlα) in the plasma in rats with acute pulmonary thromboembolism(PTE), and to explore the effects of ligustrazine. Methods Rats were randomly divided into blank group, PTE model group, ligustrazine-treated group. The PTE model in rats was established by intravenous injection of autologous blood clots. The pathological changes of the lungs were examined under a light microscope. By using immunohistochemistry, the expression levels of ET-1, COX 2 and CD54 in lung tissue was detected. The concentrations of TXB2 and 6-K PGFla in the plasma were determined by radioimmunoassay. Results Histopathological examination revealed severe lung injury in PTE group, but mild in ligustrazinc treated group. After 7 days, the levels of ET-1, COX 2 and CD54 protein expression in the lung tissue and the ratio of TXB2 to 6-K PGF1α concentrations(T/P) in the plasma were significantly higher in PTE group than in blank group(P〈0.05), but there was no significant difference between blank group and ligustrazinetreated group(P〉0.05). Conclusion Ligustrazine can reduce the expression levels of ET-1, COX-2 and CD54 in rats with acute PTE and alleviate the inflammatory reaction. Traditional Chinese medicine should be considered in acute PTE therapy.
