摘 要:目的比较不同营养治疗方式对不能经口进食的昏迷住院患者营养状况的影响。方法选取110例不能经口进食的昏迷住院患者,分为静脉营养组(PN组,21例)、肠内营养组(EN组,40例)和静脉+肠内营养组(PN+EN组,49例),3组予等氮、等能量,观察营养治疗前及营养治疗7~10 d后患者的营养状况,并比较不同营养治疗方式的治疗效果。结果营养治疗后,3组间总蛋白、白蛋白和视黄醇结合蛋白的差异均无统计学意义(P值均>0.05),PN组及PN+EN组前白蛋白水平均显著高于EN组(P值均<0.05),而血红蛋白水平显著低于EN组(P值均<0.05)。结论对于肠功能存在的昏迷患者,静脉+肠内营养治疗是较为合理的营养模式。Objective To observe the effects of different nutritional interventions on the nutrition status of inpatients in coma. Methods Twenty-eight inpatients who were unable to take food by mouth were randomly divided into three groups: parenteral nutrition group(11 patients, PN group), enteral nutrition group(8 patients, EN group) and TPN+EN group (9 patients). The three groups were given equal nitrogen and equal calory. Data of nutrition status of patients were compared before and 10 days after nutritional intervention. Results No significant differences were observed in total protein (TP), albumin, prealbumin, retinol binding protein and hemoglobin between the three groups 10 days after nutritional treatment. The levels of prealbumin in PN and PN+EN group were significantly higher than that of the EN group (both P 〈 0.05), and their levels of hemoglobin were significantly lower than those of the EN group (both P 〈 0.05). Conclusion Enteral nutrition combined with parenteral nutrition is a reasonable method for patients with intestinal function. (Shanghai Med J, 2007, 30:914-916)
