摘　　要：目的探讨三维适形放射治疗（3D—CRT）治疗局限性前列腺癌的疗效与毒副反应。方法32例局限性前列腺癌患者接受MD7745直线加速器治疗，靶区剂量2Gy／次，1次／d，5次／周，6～8周完成；靶区总剂量为60～76Gy，中位剂量72Gy。结果2年生存率为93．8％（30／32），无病生存率为81．3％（26／32），前列腺特异抗原〈1μg／L有28例。复发与转移：前列腺局部复发率12．5％（4／32），骨转移率3．1％（1／32）；32例患者分别有7例和5例膀胱毒性反应Ⅰ级和Ⅱ级，分别有6例和3例直肠毒性反应Ⅰ级和Ⅱ级，未见膀胱或直肠Ⅲ级毒性反应。结论局限性前列腺癌的三维适形放疗可提高前列腺局部剂量，相邻组织器官的毒副反应发生率低，有较高的局部控制率和良好的耐受性，可望成为局限性前列腺癌标准治疗方式。Objective To investigate the results and side-effects of prostate cancer treated with three-dimensional conformal radiotherapy（3D-CRT）. Methods 32 patients were treated with 3D-CRT.All patients were treated with a median dose of 72Gy（60-76Gy）, 2Gy/f, 5 fractions/W. Results The 2-year overall survival rate was 93.8 %. The incidence of late urinary toxicity was low, with 7 patients showing grade 1,5 patients showing grade 2 and no one showed grade 3 toxicity. In gastro-intestinal toxicities, 6 patients showed grade 1,3 patients showing grade 2 and grade 3 toxicity weren＇t found. Conclusion 3D-CRT can decrease the side-effects in the neighbor organs. A high dose to the prostate can be given using this technique for a good local control in the management of localized prostate cancer, The good tolerance and higher quality of life can be seen for all the patients.
