腮腺肿瘤的CT诊断价值

摘　　要：目的分析各类腮腺肿瘤的CT表现，评价其临床应用价值。方法回顾分析经手术病理证实的腮腺肿瘤43例，其中良性肿瘤22例，包括多形性腺瘤10例，腮腺囊肿3例，腺淋巴瘤2例，淋巴管瘤3例，神经鞘瘤2例，脂肪瘤2例；恶性肿瘤21例，包括黏液表皮样癌6例，腺泡细胞癌2例，腺样囊性癌3例，腺癌2例，恶性混合瘤4例，转移瘤2例，淋巴瘤2例。结果良性肿瘤形态规则，无侵袭性，界限清楚，密度均匀，周围邻近结构清晰；恶性肿瘤有不同程度的侵袭性，多呈分叶状或形态不规则，瘤体坏死、液化、囊性变而密度不均匀，邻近结构不清。结论CT检查能评估腮腺肿瘤的部位、大小、良恶性以及肿瘤的周围侵犯情况，基本可以满足术前临床要求，对决定手术方案，确定进一步治疗方法具有重要价值。Objective To analyze the CT manifestation of various parotid neoplasms and assess the value of CT in clinic. Methods 43 patients with parotid neoplasm proved pathologically were analyzed retrospectively, in them,benign tumors 22 cases including pleomorphic adenoma 10, cyst of parotid gland 3, adenolymphoma 2, lymphangioma 3,neurilemmoma 2 ,and lipoma 2;malignant 21 including myxoepidermocarcinoma 6, acinic cell carcinoma 2, adenoid cystic carcinoma 3, adenoma 2, malignant mixed tumor 4, metastatic carcinoma 2 and lymphoma 2. Results Benign tumors were regular,homogeneous and uninvasive with clear margin and tissue adjacent to them;malignant were invasive,irregular or multi-leaf, heterogeneous, resulting from necrosis, liquefaction and cystizing of tumors,and instinct adjacent tissue. Conclusion CT can asses the location,size, benign or malignant and invasion of adjacent tissue or organ of the parotid neoplasms,satisfy the preoprative requirement of clinic, so CT has important value in deciding project of operation projects and further therapeutic methods.
