摘 要:目的:酒精性股骨头坏死发病机制中骨髓间充质干细胞可能起作用。实验采用流式细胞仪检测酒精性股骨头坏死患者股骨头颈及股骨干骺端骨髓间充质干细胞的细胞周期,观察其增殖活性。方法:选择2005-02/2005-05在中日友好医院骨坏死与关节保留重建中心经临床及影像学确诊的酒精性股骨头坏死患者10例作为实验组(ARCO分期Ⅱ,ⅢA),男8例,女2例,平均36.9岁(28~46岁);对照组:无股骨头坏死,无酒精饮用史的新鲜股骨颈骨折患者(<1周)11例,男8例,女3例,平均60岁(55~65岁)。在患者知情同意条件下,手术抽取股骨头颈部骨髓血3~5mL。采用密度梯度离心法分离人股骨头骨髓间充质干细胞,再经贴壁筛选法筛选,采用流式细胞仪法测定第2代骨髓间充质干细胞生长增殖水平及细胞周期。结果:21例受试者均进入结果分析。流式细胞仪测定的细胞周期结果显示,与对照组比较,实验组G0/G1细胞比例升高,S+G2/M期细胞比例降低,增殖指数降低,两者差异有显著性意义(P<0.05)。结论:①酒精性股骨头坏死的股骨干近端骨髓间充质干细胞增殖活性下降。②酒精可能是通过降低骨髓间充质干细胞的增殖活性或增殖分化能力及改变其分化方向从而引起骨坏死。AIM: Mesenchymal stem cells (MSCs) can play important roles in the pathogenesy of alcohol-induced osteonecrosis of femoral head. This study was aimed to detect MSCs cycle and proliferation activity in the region of proximal femoral head and neck and femoral metaphysis of the patients with alcohol-induced osteonecrosis with flow cytometry (FCM). METHODS: From February to May 2005, the research was performed in Bone Circulation and Osteonecrosis Institute of China-Japan Friendship Hospital. Totally 10 patients diagnosing as having alcohol-induced osteonecrosis of femoral head were enrolled in the experimental group (ARCO Ⅱ ,Ⅲ A), including 8 males and 2 females, averagely 36.9 years (28-46 years). Eleven femoral neck fracture (less then 1 week) patients without the history of osteonecrosis of femoral head and alcohol drinking were enrolled in the control group, including 8 males and 3 females, averagely 60 years (55-65 years). The bone marrow blood (3-5 mL) was collected with the agreement of patients. MSCs were harvested and centrifugated by density gradient centrifugation, and then selected by the adhesive method. Proliferation and cycle of second-generation MSCs were detected with FCM. RESULTS: Totally 21 testees were involved in the result analysis. The results indicated that percentage of cells at the G0/G1 phase was higher, but lower at the (S+G2/M) phase and the proliferation ability was reduced in the experimental group as compared with the control group (P 〈 0.05). CONCLUSION: (1)The proliferation ability of MSCs may decrease in the alcohol-induced osteonecrosis of femoral head. (2)Alcohol can result in osteonecrosis by decreasing proliferation activity or differentiation of MSCs to change the differentiating direction.
