彩色多普勒超声结合美兰染色在乳腺癌前哨淋巴结检测中的临床价值探讨

摘　　要：目的探讨彩色多普勒超声结合美兰染色在乳腺癌前哨淋巴结(SIN)检测中的临床价值。方法82例乳腺癌患者随机均分为两组,组1为术中单纯美兰染色定位SLN,组2为术前彩色多普勒超声定位结合术中美兰染色法定位SLN,比较两组对SLN的检出率。利用彩色多普勒超声对组2的41例乳腺癌患者进行SLN良恶性的判定,并与术后病理结果相对照。结果组2法对SLN的检出率为92.7%,组1单纯美兰染色定位法检出率为73.2%。病理证实彩色多普勒超声对41例乳腺癌患者SLN良恶性判定的准确性为90.2%。结论彩色多普勒超声结合美兰染色在乳腺癌SLN检出及性质判定中具有较高的临床价值。Objective To explore the clinical value of color Doppler ultrasonography combined with methylene blue（MB） staining in detection of sentinel lymph node of breast cancer. Methods Eighty-two patients with breast cancer were divided into two groups. Detection rate of SLN was compared between MB staining and color Doppler ultrasonography combined with MB staining. The benign and malignant SLNs were differentiated with color Doppler ultrasonography, the results were compared with pathology. Results The detection rate of SLN was 92.7 % based on preoperation color Doppler ultrasonography and 73.2 % based on MB injection during operation. The differential diagnostic accuracy of SLN based on color Doppler ultrasonography was 90.2% in 41 patients confirmed by pathology. Conclusion Color Doppler ultrasonography combined with MB staining has great clinical value in SLN biopsy and diagnosis of breast cancer.
