摘  要:目的提供西安地区9-12岁青少年腰椎骨密度(BMD)的正常参考值,研究维生素D受体基因(VDR)多态性与9-12岁青少年BMD的关系。方法收集排除影响骨代谢疾病的西安地区9-12岁汉族儿童200例,进行问卷调查、体格测量;用双能X线骨密度仪(DEXA)测量腰椎BMD;用聚合酶链反应-限制性片段长度多态性方法(PCR-RFLPs),分析VDR基因的2个限制性酶切位点BsmⅠ、FokⅠ的多态性分布。结果①各组青少年腰椎BMD值均随年龄增长而逐渐增高,且与年龄呈显著正相关(P<0.001)。9-10岁男、女性之间BMD无明显差异(P>0.05),11岁以后女性BMD高于男性(P<0.05);②各组青少年身高(P<0.001,r=0.418-0.582)、体重(P<0.05,r=0.627-0.816)与BMD均呈明显正相关;③ff基因型BMD低于Ff与FF基因型,分别为(0.631±0.064)g/cm2、(0.654±0.012)g/cm2和(0.689±0.058)g/cm2,差异有统计学意义(F=9.24,P<0.05)。结论VDR基因在FokⅠ酶切位点的多态性与9-12岁汉族青少年BMD相关。Objective To observe the bone mineral density (BMD) of Han 9-12 children aged from 9 to 12 years old in Xi'an, and the relation between vitamin D receptor gene polymorphism and bone mineral density. Methods Totally 200 cases of 9- 12 year-old Han children without bone metabolism diseases were randomly selected. Questionnaire investigation and physical measurement of height and weight were made. BMD was measured by dual energy X- ray absorptiometry. VDR gene polymorphism (cleavaged by restriction endonuclease of Bsm Ⅰ and Fok Ⅰ ) was analyzed by PCR-RFLP. Results (1) BMD in lumbus was increased with age for both genders, and a positive correlation between bone mass and age was observed (P〈0. 001). There was no significant difference in BMD of children between females and males under 11 years old, but females had a higher BMD than males between the ages of 11 and 12 years (P〈0.05); (2) BMD in lumbus was significantly correlated with height (P〈0. 001, r = 0. 418- 0. 582) and weight ( P〈0.05, r = 0. 627 - 0. 816) (3) BMD of the ff genotype (0. 631 ± 0.064) was lower than that of genotype Ff (0. 654±0. 012) and genotype FF (0. 689±0. 058) with a significant difference (P 〈 0. 001 ). Conclusion The VDR gene polymorphism in the site cleavaged by restriction endonuclease of Fok Ⅰ is related to BMD of Han children aged 9-12 years.
