摘 要:目的探讨瘦素、胆囊收缩素、脂代谢与胆固醇结石的相关性,为胆固醇结石的成因补充理论依据。方法比较检测35例正常人与119例GS患者之间的BMI值及血清leptin、CCK、TC、TG、HDL、LDL、APOA、APOB和LP(a)水平。分析GS组内BMI和leptin与其他指标的相关性。结果GS组BMI及血清leptin、CCK、TC、TG和APOA水平显著高于正常对照组(P<0.01);GS组血清HDL水平显著低于正常对照组(P<0.01);GS组内BMI与leptin、CCK、TC、TG和APOA均呈正相关(均P<0.05);与HDL呈负相关(P<0.05);leptin与CCK、TC、TG和APOA均呈正相关(均P<0.05);与HDL呈负相关(P<0.05)。结论BMI及血清leptin、CCK、TC、TG和APOA明显升高者,具有易患胆固醇结石的趋势;leptin可能通过影响血脂代谢参与胆固醇结石的形成。[Objective] To investigate the correlation of leptin, cholecystokinin, metabohsm of lipids and cholesterol cholelithiasis, and to reinforce cause of cholesterolcalculus. [Method] BMI, serum leptin, cholecystokinin, total cholesterol, triglycerides, high density lipoprotein, low density lipoprotein, apolipoprotein a, apolipoprotein b, and lipoprotein(a) were detected in 119 patients with gallstone (GS group) and 35 normal control(NM group), and the levels of these index were compared between the two groups. In gallstone group, Pearson correlation between these indexes were evaluated. [Results] BMI, the levels of leptin, CCK, TC, TG, and APOA were significantly higher in GS group than in NM group(P〈0.01); the level of HDL was significantly lower in GS group than in NM group(P〈 0.01 ). In is group, there were positive correlations between BMI, and leptin, CCK, TC, TG or APOA (P〈0.05), and there was a negative correlation between BMI and HDL(P 〈0.05); there were positive correlations between lepitn and CCK, TC, TG or APOA(P〈 0.05), and there was anegative correlation between leptin and HDL(P〈 0.05). [Conclusions] People who have higher BMI, higher levels of leptin, CCK, TC, TG and APOA may have higher risk of developing cholesterol cholelithiasis; leptin may take part in developing cholesterol cholelithiasis through affecting metabolism of lipids.
