摘 要:目的:观察Wistar大白鼠腭部手术后裸露硬腭骨创面的组织学愈合情况。方法:选用24只Wistar大白鼠,随机分为正常对照组、实验组及实验对照组,每组8只。正常对照组不作任何处置。实验组,模拟改良的Langenbeck术式行腭成形术,在腭部中线区造成一个纵行类梭形黏骨膜缺损,然后在两侧近磨牙腭侧龈缘处做松弛切口,松弛切口区遗留的裸露骨面暴露在口腔中。实验对照组实施同样的手术,但在腭部中线区不造成黏骨膜缺损,仅于中线及两侧近磨牙腭侧龈缘做切口,将黏骨膜瓣翻起后再复位缝合。术后8周处死动物,切取硬腭作组织学切片,观察不同处理措施的腭部组织学愈合情况。结果:实验组,松弛切口区遗留的裸露骨面由瘢痕结缔组织修复,上皮下由大量较粗的胶原纤维构成,无疏松结缔组织,胶原纤维穿入腭骨、牙槽骨及牙龈中。实验对照组,上皮下充满胶原纤维,深层结缔组织较疏松,未见胶原纤维穿入腭骨、牙槽骨及牙龈中。结论:腭部手术后遗留的裸露骨面由瘢痕组织修复,其胶原纤维穿入腭骨及牙槽骨中,这可能是影响上颌骨正常生长发育的重要生物学基础。Objective: To observe the healing process of palate wound with denuded bone following palatal surgery in the rats. Methods: 24 Wistar rats were selected and divided into 3 groups randomly. The rats in group 1 were the control without any intervention. An elliptical mucoperiosteal flap was excised out in the midsagittal region of the palate for the rats in group 2, relaxation incisions were made and the mucoperiosteum was elevated and closed at the midline, leaving two wounds of denuded bone adjacent to teeth. The rats in group 3 were excised at the midline and two posterior teeth regions of palate, the mucoperiosteum was then elevated , replaced and sutured, no bone was denuded. Animals were killed at the 8th week after the surgery. Palatal tissue were observed by a light microscopy. Results: In group 2, the areas of denuded bone were repaired by scar tissue, surfaced by a layer of thinner epithelium, rete pegs either tall or short, either coarse or thin. A lot of coarse collagenous fibres appeared, some of them penetrating into palatal bone; loose connective tissue were not found. No Sharpey's fiber was found in group 3. Conclusion: The maxillary growth and development may be influenced by palatal surgery, and the formation of scar tissue is one of the most important biological factors.
