低场MRI平扫在诊断进展期胃癌胃壁及胃周浸润上的价值

摘　　要：目的探讨低场MRI平扫在进展期胃癌胃壁及胃周浸润上的诊断价值。方法使用MRI扫描仪对65例病理证实的进展期胃癌患者进行扫描并对照病理结果。结果以胃周脂肪间隙征象、浆膜轮廓征象和胃周低信号带征象判定AGC浆膜面受侵,其准确率分别为60.00%、84.62%和78.46%,后两者的准确率较高(P<0.05)。本组有9例分别侵犯了胰腺、横结肠及其系膜、脾脏,依据以上标准均获得诊断,诊断准确率100%。分别应用SE/T1WI、FSE/T2WI序列、GRE/T1WI序列判定AGCT分期准确率分别为58.46%、63.08%、63.08%。结论应用低场MRI对进展期胃癌胃壁及胃周浸润进行判定,是适合我国国情的诊断方法。Objective To evaluate the value of low-field-intensity MRI in diagnosing gastric wall and perigastric invaded by advanced gastric carcinoma（ AGC ）. Methods Use 0.5T MRI made by Elscint Company to scan 65 AGC and compare the MRI results with pathotogical findings. Results The accuracy rates were 60.00%, 84.62% and 78.46% respectively by fat interval sign, localized serous sign and low belt sign in serous. The accuracy of the last two is higher than that of the first one（P〈 0.05）. The accuracy of judging perigastric invaded by AGC was 100%, though there were only 9 cases. The accuracy rates were 58.46%, 63.08% and 63.08% respectively by sErrlwI, FSE/T2WI and GRE/TIWI（P〉0.05）. Conelusion It is fit for our economic situations to use low-magnetic-intensity MRI to evaluate the degree how AGC has invaded the gastric wall and perigastric.
